Normative and self-perceived orthodontic treatment need of a Peruvian university population

Abstract: Background: Previous studies on orthodontic treatment need in young adults have shown that up to 50% had malocclusions that needed orthodontic treatment. The aims of this study were to assess the normative and self-perceived need for orthodontic treatment using the Index of Orthodontic Treatment Need (IOTN) and to determine if the treatment need levels were influenced by sex, age and socio-economic status (SES) in a sample of Peruvian young adults.

“…That figure was similar to that reported in the only previous population-based study carried out among 11-12-year-old Thai children (20.3%) [12]. At present, orthodontic care in Peru is only provided by the fee-for-service modality, which makes it expensive and unaffordable for most people [19,24]. Since orthodontic treatment is not offered in public health services, different expectations about malocclusion and its treatment can be expected.…”

“…We used self-perceived malocclusion rather than a normative definition of malocclusion since normative orthodontic need indexes are not strongly associated with people's perceptions of their oral health status [19,20] and quality of life [3,14,15]. Subjective impacts directly identified as being related to malocclusion have been previously used to capture subjective feelings [21,22].…”

“…Also, as indicated by lower negative LR (less than 0.5), for not requiring orthodontic or restorative need, self‐reports have moderately good diagnostic accuracy. This finding is consistent in literature with perceived orthodontic treatment need and normative need correlating well . As well, with permanent dentition and skeletal maturation in adulthood, awareness of orthodontic treatment needs may be better assessed in permanent compared to mixed and developing dentitions in adolescents; however, comparisons across age groups have been seldom explored .…”

“…One possible explanation is that the adolescent population has always been particularly concerned with appearance and aesthetics, sometimes not in a mature or objective way, and this could influence the results (16,17). On occasion, the patient’s perception of his or her malocclusion is not related to standard treatment need as determined objectively by the indices (16,18,19). Moreover, patients’ perceptions of malocclusion are usually quite different from those of the specialist (18,20).…”
