Background: Rhinoplasty is widely recognized as one of the most difficult procedures in plastic surgeries, two-dimensional (2D) and three-dimensional (3D) digital imaging have played an important role in this field, being as a valuable tool throughout preoperative and intraoperative analysis to evaluate the use and efficacy of preoperative 3D virtual simulation and surgical planning in rhinoplasty. Methods: This prospective randomised clinical work was performed on 30 patients with external nasal deformities of dorsum with or without tip deformities as dorsal hump, Nasal tip jobs, posttraumatic deformities. Participants had been subdivided into two groups equally: Group A: Conventional-procedure rhinoplasty and group B: Rhinoplasty with preoperative 3D-simulation. All patients were subjected to software CT scanners (giving the comprehensive 3-D digital visualization for the anatomy of the head and neck of the patient). Results: Rhinoplasty outcome evaluation (ROE) questionnaire demonstrated that participants who had been satisfied with the look of their nose as well as the aesthetic score were significantly higher in group B. Surgical time had been significantly increased in group A. There was a significant agreement between the 3 raters in group B and no significant variation was existed among both groups as regard to complications, the average objective evaluation of the rhinoplasty, breathing from nose and functional score between the studied groups. Conclusions: In rhinoplasty, virtual simulation using 3D technology and 3D printed anatomic surgical guide showed better promising results regarding surgical time, ROE and patients' satisfaction with more reliable results compared to conventional methods. Virtual simulation assists in harmonizing the aesthetic objectives of the patient and surgeon.

Surgical rhinoplasty (SR), commonly known as nose job, is a widely practiced cosmetic surgery globally, aimed at addressing diverse aesthetic and functional concerns related to the nose. In recent years, non-surgical rhinoplasty (NSR) has gained popularity due to advanced techniques involving hyaluronic acid (HA) dermal fillers, offering advantages such as affordability, reduced side effects, and faster results. However, concerns persist about the suitability of dermal fillers for nasal anatomy and potential complications, prompting this comprehensive review. This study systematically evaluated the techniques, fillers, safety, and patient satisfaction associated with NSR, with the intent of providing valuable insights for clinicians and patients considering NSR or SR for improved aesthetic outcomes. The literature search, following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria, yielded 16 relevant studies from an initial pool of 1002 articles. These studies covered various aspects of NSR, including techniques, complications, limitations, and positive results. In conclusion, NSR appears to be a quick and safe option for addressing minor nose shape issues, particularly through the use of HA fillers, but further discussion and standardization are necessary to address risks and limitations. A randomized controlled trial (RCT) using photographic evidence could significantly propel the progress of this evolving treatment. RCTs offer an optimal method to assess NSR's adverse effects and overall outcomes by allowing controlled comparisons between treatment and control groups. This approach minimizes biases and generates reliable statistical data, which is critical for evaluating safety, efficacy, and potential risks, thereby guiding informed clinical decisions.

The Role of Autologous Fat Grafting in Rhinoplasty

Rhinoplasty is one of the most popular aesthetic surgeries worldwide and often includes grafting techniques to achieve optimal results. One of these grafting techniques is autologous fat transfer, which has been used to increase volume, camouflage irregularities, and/or improve the quality of the nasal skin soft tissue envelope. Moreover, minimally-invasive approaches for altering the nasal appearance have recently increased and become known as "liquid" or "non-surgical rhinoplasty." These non-surgical approaches include altering the nasal appearance with filler injection to induce volumetric changes in lieu of extensive surgery. The use of fat grafting as a filler is favorable to achieve well-balanced aesthetic results without compromising the nasal skin soft tissue envelope. This capability is partly because of the regenerative potential of fat grafts, serving to improve the quality of surrounding soft tissues. In contrast, commercial injectable fillers are inert. This article highlights the role of fat grafting in surgical and non-surgical rhinoplasty to provide surgeons with an overview of the potential of these vastly abundant, biocompatible, and cost-effective grafts.
