Nonsecretory myeloma.Immunofluorescent demonstration of paraprotein within bone marrow plasma cells

Abstract: A patient with multiple myeloma is reported with hypogammaglobulinemia and the absence of a paraprotein in the serum, and absence of Bence Jones protein in the urine. The clinical course was malignant with massive plasma cell infiltration of the liver and kidneys contributing to insufficiency of each organ. Immunofluorescent studies on bone marrow smears demonstrated bright staining for kappa and weak staining for gamma antigenic determinants within the cytoplasm of the plasma cells. It is suggested that the a… Show more

“…Clinical, radiological and haematological features in non-excretory disease do not appear to differ significantly from those in excretory disease. Elevated ESR levels may have more significance in non-excretory disease, reflecting coexistence of complications and indicating a poor prognosis (Arend & Adamson, 1974;Fabia et al, 1974). Uraemia, as in our cases, is almost always associated with hypercalcaemia and is usually reversible with correction of the hypercalcaemia (Prost et al, 1973).…”

“…Although some of these immunofluorescence studies suggest the presence of free immunoglobulin chains within the cell, they do not, as pointed out by Arend and Adamson (1974), constitute unequivocal evidence for this.…”

“…Conflicting reports exist regarding the natural history of non-secretory myeloma, indicating both a more and a less favourable prognosis (12, IS). Im-munofluorescence and immunoperoxidase studies of intracellular immunogloblslins in myeloma cells suggest that patients with non-secretory myeloma can be subdivided into two categories: those who do not synthesize and those who synthesize but do not secrete monoclonal immunoglobulin (2,3,5,6,9,18,20,21,22,23,25,26,28,29,31,34).…”
