Nonoperative Treatment of an Os Peroneum Fracture in a High-level Athlete: A Case Report

Abstract: Background The os peroneum is a sesamoid bone in the peroneus longus tendon. Fractures of the os peroneum are rare. Some authors recommend surgery for active patients. Case Description A 41-year-old male professional tennis coach sustained a minimally displaced fracture of the os peroneum. He was treated with restricted weightbearing for 2 weeks, followed by physical therapy and gradual return to activities. He returned to tennis 8 weeks after injury. Followup 7 years after the injury showed he had full streng… Show more

“…The use of magnetic resonance imaging (MRI), singlephoton emission computed tomography (SPECT), scintigraphy or ultrasound has proven useful in imaging this area and for differential diagnosis (Crain & El-Khoury, 1989;Lazos, 1995;Rademaker et al 2000;Okazaki et al 2003;Brigido et al 2005;Wang et al 2005;Zgonis et al 2005;Fernandes et al 2007;Sammarco et al 2010;Sofka et al 2010;Chadwick et al 2011). The variety of pathological entities described in this area include fracture of the os peroneum (Stropeni, 1920;Hadley, 1942;Brav & Chewning, 1949;Grisolia, 1963;Peacock et al 1986;Wilson & Moyles, 1987;Cachia et al 1988;Pessina, 1988;Hogan, 1989;Peterson & Stinson, 1992;Wander et al 1994;MacDonald & Wertheimer, 1997;Bessette & Hodge, 1998;Requejo et al 2000;Okazaki et al 2003;Smith et al 2011), hypothetical fracture of the cuboid bone (Haguier, 1937;Ginieys, 1939), peroneus longus tendon rupture (Burman, 1956;Evans, 1966;Davies, 1979;Peacock et al 1986;Thompson & Patterson, 1989, 1990Pai & Lawson, 1995;Sammarco, 1995;Truong et al 1995;…”

“…Patients who are able to relate an initiating injury are defined as acute; whilst patients who were unable to are classified as chronic[6,10]. Non-surgical management rarely yield acceptable outcomes[11,12]. Due to the PLT’s role in mid and hindfoot stability, surgical correction is often warranted with most authors achieving reproducible results.…”
