Noninvasive Procedures to Evaluate Liver Involvement in HIV‐1 Vertically Infected Children

Abstract: : More than half of our population of HIV-infected children had biological and/or radiological signs of liver affection. Regular follow-up of liver function is necessary in these patients, which is now possible with noninvasive procedures.

“…Success rates were lower in younger children (14,19,21,23,43,44). Boys had higher liver stiffness values compared with girls in two studies (21,45), but in six studies gender had no effect on measurements (19,22,23,37,41,42).…”

“…Transient elastography of the liver was performed using Fibroscan® (ECHOsens S.A.S.U, Paris, France) as a noninvasive ultrasound modality to assess liver fibrosis, as previously shown in different liver conditions including HIV-related liver disease in children. 20,21 A single pediatric radiologist (EED) reviewed the ultrasounds at the time of initial diagnosis of hepatic abnormality.…”

“…Using FibroTest, we have already estimated the prevalence of fibrosis stages in hyperlipidemic patients [31], diabetic patients [32], and the French general population over the age of 45 years [20]. Epidemiological studies (independent of the inventor) using FibroTest without biopsy were also performed to assess fibrosis in patients with hemophilia [33,34], HIV [35], and Crohn's disease [36], to assess the impact on fibrosis of physical activity in NAFLD [37], the impact of methotrexate on psoriasis [38], the association between genetic polymorphism and fibrosis in type 2 diabetics [39], and the association between serum testosterone level and fibrosis in men with CHC [40].…”
