Background: Pulmonary hypertension (PH) is a well-known complication of chronic obstructive pulmonary disease (COPD). It remains unclear whether exercise parameters can be used to discriminate between COPD patients with associated PH (COPD-PH) and COPD patients without associated PH (COPD-nonPH). Objective: To study whether the existence of pulmonary hypertension in COPD is related to characteristic findings in gas exchange and circulatory parameters during cardiopulmonary exercise testing (CPET). Methods: We retrospectively analyzed CPET data in 25 COPD patients in whom right heart catheterization had been performed. Differences were assessed between COPD-PH and COPD-nonPH patients in peak oxygen uptake (VO₂ peak), ventilatory efficiency (VE/VCO₂), oxygen pulse, maximal ventilation and pulse oximetry (S_pO₂). Results: PH was found in 10 of 25 patients (mP_pa = 33 ± 7 mm Hg), in 15 patients mean pulmonary artery pressure (mP_pa) was below 25 mm Hg (18 ± 3 mm Hg). CPET in COPD-PH was characterized by a higher VE/VCO₂ at nadir, a higher VE/VCO₂ slope, and a lower S_pO₂ at rest and during exercise, but values in both groups were overlapping considerably. In the whole group mP_pa was associated with resting P_aO₂ (r = –0.70, p < 0.001), VE/VCO₂ nadir (r = 0.43, p < 0.05), and inversely related to S_pO₂ at rest and during exercise (r = –0.58 and r = –0.64, p < 0.01, respectively). Conclusion: Although CPET characteristics showed a large overlap in both groups, the existence of PH in COPD is associated with a significantly reduced ventilatory efficiency during CPET. However, a low S_pO₂ at rest and a further decrease during exercise similarly suggest the presence of PH in COPD.

Ventilatory thresholds (VTs) are key physiological parameters to evaluate physical performance and determine aerobic and anaerobic transitions during exercise. Current assessment of these parameters requires ergospirometry, limiting the evaluation to laboratory or clinical settings. In this work, we introduce a wearable respiratory system that continuously tracks breathing during exercise and estimates VTs during ramp–tests. We validate the respiratory rate and VTs predictions in 17 healthy adults using ergospirometry analysis. In addition, we use the wearable system to evaluate the VTs in 107 recreational athletes through ramp tests outside the laboratory and show that the mean population values agree with previous studies in large groups. We envision that respiratory wearables can be useful in determining aerobic and anaerobic parameters outside clinical setting with promising applications in health telemonitoring and human performance.

Physical exercise can improve sputum clearance in patients with cystic fibrosis (CF). To set up individual training protocols it is desirable to know the anaerobic threshold (AT). Established methods such as blood lactate measurements and ergometry can only be performed in specialized centers. Conconi showed that the heart rate threshold (HRT), i.e., the deflection point from the linear relationship between work load and heart rate, correlated significantly with the AT in healthy adults. To assess the reliability of the HRT in CF, we performed ergometry in 32 CF patients (mean age, 21.0 ± 5.5 years; mean Shwachman score, 77.8 ± 12.0) according to the Conconi protocol. The HRT was compared with the aerobic threshold (AeT) as determined by the V‐slope method and with two turn points in the lactate performance curve (LTP1, LTP2). An HRT could be obtained in only 17 of the 32 patients (53%). In these 17 patients there was a significant correlation between HRT and the other thresholds, but the absolute values for the AT differed considerably: The mean HRT was 132% higher than the AeT according to Beaver, 107% higher than LTP1, and 19% higher than LTP2. Exercise protocols that rely solely on the HRT in CF will lead to excessive exertion during exercise training programs in these patients. According to these results the HRT of Conconi is not a suitable method to determine appropriate exercise levels in CF training programs and might even be harmful in CF patients. These results also indicate the need to test the reliability of a diagnostic procedure that has been developed only for healthy people. © 1998 Wiley‐Liss, Inc. Pediatr. Pulmonol. 1998; 25:147–153.
