This paper presents a novel blood pressure (BP) estimation method based on pulse transit time (PTT) and pulse arrival time (PAT) to estimate the systolic blood pressure (SBP) and diastolic blood pressure (DBP). A data acquisition hardware is designed for high-resolution sampling of phonocardiogram (PCG), photoplethysmogram (PPG) and electrocardiogram (ECG). PCG and ECG perform as the proximal timing reference to obtain PTT and PAT indexes, respectively. In order to derive a BP estimator model, a calibration procedure including a supervised physical exercise is conducted for each individual which causes changes in their BP and then, a number of reference BPs are measured alongside the acquisition of the signals per subject. It is suggested to use a force-sensing resistor (FSR) that is placed under the cuff of the BP reference device to mark the exact moments of reference BP measurements, which are corresponding to the inflation of the cuff. Additionally, a novel BP estimator nonlinear model, based on the theory of elastic tubes, is introduced to estimate the BP using PTT/PAT values precisely. The proposed method is evaluated on 32 subjects. Using the PTT index, the correlation coefficients for SBP and DBP estimation are 0.89 and 0.84, respectively. Using the PAT index, the correlation coefficients for SBP and DBP estimation are 0.95 and 0.84, respectively. The results show that the proposed method, exploiting the introduced nonlinear model with the use of PAT index or PTT index, provides a reliable estimation of SBP and DBP.Index Terms-cuff-less blood pressure, mobile health (mHealth), pulse transit time (PTT), pulse arrival time (PAT), vital signals

This article describes a multimodal cardiovascular information measurement method using a wearable device composed of piezoelectric transducers. Cardiovascular diseases are increasing with the aging population, and they constitute a significant portion of the causes of death and long-term care. In recent years, daily-life monitoring using wearable sensor devices has attracted particular attention for the prevention and early detection of cardiovascular diseases. However, recent wearable devices can only measure limited cardiovascular information such as the heart rate. In contrast, the proposed method can simultaneously measure heart rate variability, pulse wave propagation velocity, and blood flow velocity using only a piezoelectric transducer array.

Cuffless Blood Pressure Estimation Using Single Channel Photoplethysmography: A Two-Step Method

Traditional cuff-based blood pressure (BP) monitoring procedure causes inconvenience and discomfort to the users. To overcome these limitations, cuffless BP estimation based on pulse transit time (PTT) and single-channel photoplethysmography (PPG) has been proposed. However, existing studies based on PTT and PPG for BP estimation did not achieve AAMI/ISO standard criteria for BP measurement (mean difference within ±5mmHg and SD of difference within ±8mmHg) under each BP category (Hypotensive, Normotensive and Hypertensive). This study aims to validate an innovative two-step method for PPGbased cuffless BP estimation. A combined database was derived from two online databases (Queensland and MIMIC II) to cover a wide range of corresponding BPs. In total, there were 18010 raw PPG signal segments (5 seconds for each) with corresponding BPs, separated into two halves for training and testing of algorithms (independent datasets). Each PPG signal segment was pre-processed to extract 16 signal features. Later, three significant features have been selected using multicollinearity test. The traditional generic (trained with uncategorized BP) algorithm and two-step algorithm (specifically optimized for each BP category) were developed using machine learning. Generally, the two-step algorithm achieved the AAMI/ISO standard in estimating systolic BP (mean ±SD: 0.07±7.1 mmHg, p<0.001) and diastolic BP (−0.08±6.0 mmHg, p<0.001). Categorically, the two-step method also achieved standard accuracy in all BP categories except Hypotensive systolic BP whereas generic algorithm did not conform to standard accuracy in any BP category except Hypotensive diastolic BP and Normotensive categories. Compared to the traditional generic algorithm, the two-step algorithm specifically designed for three different BP category patients and achieved standard accuracy for cuffless BP estimation.
