Nonhealing corneal ulcers in cats: 29 cases (1991–1999)

Abstract: Brachycephalic cats appear to be predisposed to developing nonhealing corneal ulcers. The combination of superficial debridement and grid keratotomy did not decrease mean healing time of nonhealing ulcers, compared with superficial debridement alone. Grid keratotomy may predispose cats with corneal ulcers to develop a corneal sequestrum.

“…The pathogenesis of corneal sequestrum is not understood. It is a widely accepted theory that corneal insult is an important initiating factor 14,15,26,36 . This was supported in this study by observations of breed‐related conformation, antecedent and concurrent ocular disease, and previous therapeutic interventions.…”

“…Chronic nonspecific ocular disease and specific causes of ocular irritation were present in more than half of cases. Procedures to stimulate corneal healing in indolent ulceration 36,47–49 and phenol cautery had been performed in 27% of cats. Such procedures represent a form of iatrogenic trauma to the cornea and appear to facilitate sequestrum formation 13,36 .…”

“…Based on these findings, it was concluded that SCCED relates to inherent abnormalities in epithelial basement membrane structure, adhesion complexes and adjacent anterior stromal extracellular matrix. While similar disease manifestations have been observed in cats and horses, they are comparatively poorly described, and a common histopathological description has not been illustrated. The purpose of this retrospective study was to delineate the histopathological features of chronic, nonhealing, superficial, corneal ulcers in the equine species.…”

“…Even today, the aetiology is unknown, although traumas, prolonged corneal irritation, alterations of the tear film and of the corneal innervation, dystrophy, infections (Herpesvirus) and genetic predisposition would appear to play an important role in determining its onset (Startup, 1988;Pentlarge, 1989;Morgan, 1994;Cullen et al, 1999;La Croix et al, 2000).…”
