BackgroundPersonal hypertension management is a cornerstone in the prevention of hypertension complications. In Eritrea, the increase in the national life expectancy rate has been accompanied by an increase in hypertension complications such as stroke. Hence, this study was designed to identify barriers and facilitates to hypertension management from the perspective of the patients.MethodsThis was a qualitative study of a total of 48 individual in-depth interviews and two focus group discussions. It was conducted among hypertensive patients who were attending outpatient services at two hospitals in Asmara, Eritrea.ResultsThis study identified barriers and facilitators of hypertension management related to the individual patient, family and community, and healthcare system. With respect to individual factors, economic barriers, stress, non-adherence to medications due to the use of traditional remedies, and difficulties and misconceptions about following physical activity guidelines were mentioned as barriers to hypertension management. Related to the community and healthcare system, low community awareness, community stigma, and inadequate health promotion materials were stated as barriers. Individual knowledge, family, and government support were reported as very important factors to the patient’s success in the personal hypertension management.ConclusionsCounseling patients about adherence to medication, strengthening family and government support, and empowering families and the community with appropriate knowledge of hypertension management could potentially help in an individual’s adherence.

Серцево-судинні захворювання (ССЗ) продовжують займати перше місце серед причин смертності працездатного населення, а їх поширеність вища в країнах з низьким та середнім рівнями доходу. В зв’язку з обмеженими фінансовими ресурсами цих країн виникає необхідність раціонального розподілу коштів, виділених на охорону здоров’я. Особливого значення в цих умовах набувають превентивна кардіологія та кардіореабілітація (КР). Мета – провести огляд наукової літератури, присвяченої економічній ефективності та організаційним аспектам реалізації програм КР у країнах з різним рівнем доходів; запропонувати модель КР для України. Матеріал і методи. Проведено огляд літератури з обраної теми в наукометричній базі даних MEDLINE та на офіційних електронних ресурсах міжнародних об’єднань кардіологів, лікарів фізичної та реабілітаційної медицини. Пошук проведено за ключовими словами: cardiac rehabilitation, cardiorehabilitation, cardiovascular rehabilitation, low/middle income country, core components, models, benefit, effectiveness, economic effectiveness. Результати і висновки. У статті проаналізовано ефективність та описано особливості організації КР у країнах з різним рівнем доходів. На основі огляду літератури зроблені висновки щодо можливих шляхів розвитку та запропонована модель організації КР в Україні. Використання альтернативних моделей КР може підвищити її доступність для більшості населення України, а, отже, покращити якість життя та знизити смертність від ССЗ.

The prevalence of noncommunicable diseases in Eritrea is increasing. Tobacco use is a recognized risk factor for most of these diseases, especially cardiovascular disease. No data have been published on tobacco use in Eritrea. The present study sought to establish the prevalence of tobacco smoking in Eritrea. The World Health Organization STEPwise approach was used for the survey, conducted in 2004 on a random national sample size of 2,460 subjects (response rate = 93.7%). The prevalence of tobacco smoking in the general population was 8.1%; the prevalence was 15% among men, compared with 0.6% among women. Prevalence rates were higher in those older than 45 years of age. The prevalence of tobacco smoking was higher among Muslims (11.4%) than Orthodox Christians (5.8%), and among alcohol drinkers (10.2%) than nondrinkers (6.6%). The majority of tobacco users (89.3%) used commercially available cigarettes. A study on knowledge, attitudes, and practices regarding tobacco use is needed to determine the behavioral factors leading to tobacco smoking among the vulnerable groups.
