Noncaseating suppurative granulomatous lymphadenitis in adult onset Still’s disease – a diagnostic dilemma in a tuberculosis-endemic region: a case report

Abstract: BackgroundLymphadenopathy is not an uncommon presentation of adult onset Still’s disease: it is present in up to two thirds of patients with adult onset Still’s disease. The characteristic appearance of lymphadenopathy is described as intense, paracortical immunoblastic hyperplasia. Changes in light microscopy may resemble lymphoma, but immunohistochemistry reveals a benign polyclonal B cell hyperplasia.Case presentationWe describe a 67-year-old Sri Lankan woman who manifested relapsing prolonged fever, raised… Show more

“…Granuloma with suppurative changes can occur due to variety of infections like fungus, mycetoma, leishmaniasis, bacteria along with atypical mycobacterium mainly marinum and fortuitum [15,16] . Cases had been reported with this morphology in noninfective immune conditions like adult onset Still's disease also [17] . The author had an experience of misdiagnosing clinical cold abscess as suppurative abscess since cytomorphology showed neutrophilic necrosis without granuloma.…”

Only 5 of 40 cases were reported between 1991 and 2000, 8 between 2001 and 2010, and the remaining 25 between 2011 and 2020 (Table 1).…”
