This article outlines an evidence-informed, interdisciplinary, multidimensional, comprehensive action plan for the American Urogynecologic Society to improve care of women with overactive bladder (OAB) while minimizing treatment-related adverse events, including cognitive impairment. It is a "call to action" to advance basic, translational, and clinical research and summarizes initiatives developed at the State-of-the-Science Conference on OAB and Cognitive Impairment to (1) develop framework for a new OAB treatment approach in women, (2) define research gaps and future research priorities, (3) champion health equity and diversity considerations in OAB treatment, (4) foster community and promote education to remove stigma surrounding OAB and urinary incontinence, and (5) elevate visibility and impact of OAB, by creating partnerships through education and engagement with health care professionals, industry, private and public payers, funding agencies, and policymakers.

Introduction: Women have a higher life expectancy than men. However, this does not guarantees a healthy life as women are often disadvantaged section of society. Additionally, women develop a series of natural physiological changes during midlife accompanied with chronic conditions including genitourinary conditions. We aimed to estimate the prevalence and determine the correlates of chronic renal failure, incontinence and kidney stones among women aged ≥45 years in India. Methods: We employed data from the first wave of Longitudinal Ageing Study in India (LASI) wave-1, 2017-2018. A sample of 32,097 women aged ≥45 years were included. Descriptive statistics were used to report prevalence along with 95% confidence interval as a measure of uncertainty. Separate multivariable regression analysis was conducted to assess the association between various socio-demographic and behavioral correlates with the outcome i.e. chronic renal failure, incontinence and kidney stones. Results: The overall prevalence of chronic renal failure was 0.53% (95% CI: 0.45-0.62), incontinence was 2.9% (95% CI: 2.72-3.09) and 2.28% (95% CI: 2.11-2.45). The multivariable regression analysis showed participants aged ≥75 years had a higher odds [AOR: 1.81 (95% CI: 1.34-2.44)] of having incontinence. Kidney stone were significantly akin [AOR: 1.98 (95% CI: 1.53-2.55)] among participants having obesity.

Associations Between Unmet Social Needs and Overactive Bladder

Purpose: There is growing interest in how social determinants of health may explain relationships between social conditions and health. Overactive bladder is common in the United States, posing a significant public health burden. It was the goal of the present study to assess whether overactive bladder was associated with unmet social needs in a large, cross-sectional sample of patients. Materials and Methods: A community-based sample of adults was recruited electronically to complete questionnaires on clinical and demographic information, urinary symptoms and unmet social needs. Multivariable logistic regression was used to assess for association between overactive bladder and multiple unmet social needs. Results: The sample of 3,617 participants had a meanAESD age of 47.9AE17.3 years. The majority of participants identified as White, non-Hispanic (83.5%), and female (77.6%). A total of 1,391 patients (38.5%) were found to have overactive bladder. The presence of overactive bladder was associated with numerous unmet social needs, including housing instability, food insecurity, worry about utilities, lack of transportation, skipping medical appointments, needing legal help, stress in personal relationships, worry about toilet access and plumbing at home, and lack of social supports. On multivariable analysis adjusting for multiple covariates, all associations between overactive bladder and social needs remained significant. Conclusions: Multiple unmet social needs were associated with overactive bladder, which may have implications for the overall management of overactive bladder patients. It is important for providers to consider these unmet social needs of patients, as highlighting and addressing these factors hopefully can help improve care of individuals with overactive bladder.
