Non-visible colovesical fistula located by cystoscopy and successfully managed with the novel Padlock® device for endoscopic closure

Abstract: A 69-year-old woman with high risk for surgery presented with recurrent cystitis, pneumaturia, and enteruria 2 months after medical-treated diverticulitis. Computerized tomography demonstrated colovesical fistula but colonoscopy could not locate the fistulous opening. A cystoscopy was performed and the fistulous tract was shown using a guidewire. Then, a novel over-the-scope clip device Padlock system was released in the sigmoid colon, with successful endoscopic closure through this not previously described co… Show more

“…One month later, cystography revealed that there was no contrast medium leakage into the rectum. Similar conservative treatment of rectovaginal fistula has been reported by Moreno-Selva et al, 13 3 reported a case of RVF that was treated with a novel over-thescope clip device Padlock V R system combined with urology. Kanehira et al 2 performed repair surgery for ten patients with RVF by transanal endoscopic microsurgery, among whom seven patients were cured and three procedures failed.…”

“…With the development of robotic surgery technology, Yeung et al 14 provided a parametric scheme for repairing RVF using robots. Recently, Velayos et al 3 reported a case of RVF that was treated with a novel over-thescope clip device Padlock V R system combined with urology. Kanehira et al 2 performed repair surgery for ten patients with RVF by transanal endoscopic microsurgery, among whom seven patients were cured and three procedures failed.…”

“…The use of OTS clips for colovesical and colovaginal fistulas has also been cited in the literature in several case reports. Velayos et al described successful use of an OTS clip (Padlock Clip, Aponos Medical, Kingston, NH, USA) to manage a colovesical fistula secondary to diverticulitis, using cystoscopic wire access similar to the technique presented in this series [16]. Several small case series of OTS clip use throughout the entire GI tract include individual patients with colovesical and colovaginal fistulas, again treated with variable success [17, 18].…”
