Non-tuberculous mycobacteria have diverse effects on BCG efficacy against Mycobacterium tuberculosis

Abstract: SummaryThe efficacy of Bacillus Calmette-Guerin (BCG) vaccination in protection against pulmonary tuberculosis (TB) is highly variable between populations. One possible explanation for this variability is increased exposure of certain populations to non-tuberculous mycobacteria (NTM). This study used a murine model to determine the effect that exposure to NTM after BCG vaccination had on the efficacy of BCG against aerosol Mycobacterium tuberculosis challenge. The effects of administering live Mycobacterium av… Show more

“…The BCG vaccine, which is to protect against tuberculosis (TB), offers different levels of protection among adult human populations (18)(19)(20)(21). The variations have been partly attributed to increased exposure to NTM species in some geographical regions (20,21), reducing protection against pulmonary TB in adults (22).…”

“…91 The findings from several other publications supported this hypothesis, that the increased efficacy of BCG vaccination was observed in locations farther from equator, where the infants suffer from lower exposure to NTMs. 22 This conflicting data provides us several competing factors to keep in mind during development and testing. First, all studies to date were all based on the measurement of immunologic responses.…”

“…Several clinical trials have proved that both M. tuberculosis and NTM infection have diverse effects on BCG efficacy against M. tuberculosis. 22,96 Thus, future studies aiming to evaluate the protective effectiveness of revaccination may wish o enroll children clearly documented to have received primary BCG vaccination, and who also have a negative PPD result to prevent the potential interference of M. tuberculosis and NTM infection.…”

“…Exposure to NTM has been suggested to impact on the efficacy of the Bacille Calmette-Guérin vaccine 16 and to exhibit cross-reactivity to the tuberculin skin test (TST), leading to increased difficulties in interpreting TST-positive results and evaluating the protection through the only available vaccine against TB.…”
