Association of gout and colorectal cancer in Taiwan: a nationwide population-based cohort study

ObjectivesThis study aimed to determine colorectal cancer (CRC) risks among patients with gout through a follow-up study on a nationwide population-based cohort that included patients with gout and the general population in Taiwan.ParticipantFrom the Taiwan National Health Insurance Research Database, we identified 28 061 patients who were newly diagnosed with gout between 2000 and 2010 as the study cohort. We randomly selected 84 248 subjects matching in gender, age and baseline year as comparison cohort. The cohorts were followed up until CRC occurrence, withdrawal from the system of National Health Insurance, or Dec. 31, 2013.Primary and secondary outcome measuresCumulative incidences and incidence rate ratios (IRRs) of CRC between two cohorts were examined. The Cox proportional hazards model was used to evaluate risk factors associated with CRC development.ResultsDuring the 13-year follow-up, the incidence rate of CRC development in the gout cohort reached 2.44 per 1000 person-years, which was higher than the 2.13 per 1000 person-years in the control cohort (IRR=1.15; 95% CI 1.04 to 1.26). After adjusting for age, gender, urbanisation status and comorbidities, including hypertension, diabetes and hyperlipidaemia, gout showed no significant association with increased risk of CRC occurrence (adjusted HR=1.03; 95% CI 0.93 to 1.14).ConclusionsSimilar risks of CRC incidence were observed in patients with and without gout in Taiwan. Allopurinol and colchicine are commonly used as urate-lowering drug and anti-inflammation medication in Taiwan and had been shown to reduce the risk of CRC incidence. Thus, further pharmaco-epidemiological studies should be carried out to specifically assess the role of allopurinol in the relationship between gout and CRC.

ObjectiveTo compare the clinical efficacy of febuxostat combined with a low‐purine diet versus allopurinol combined with a low‐purine diet in the treatment of gout.MethodsIn this prospective controlled trial, 98 gout patients admitted to our hospital from February 2021 to December 2022 were enrolled as study subjects. Patients were randomly assigned to the study group (febuxostat combined with a low‐purine diet) and the control group (allopurinol combined with a low‐purine diet), with 49 patients in each group. The therapeutic effect was evaluated based on joint function and serum uric acid levels after treatment, and classified into three levels: markedly effective, effective, and ineffective. The levels of inflammatory factors, including tumor necrosis factor‐a (TNF‐a), cytokine interleukin‐1beta (IL‐1β), and interleukin (IL)‐18 (IL‐18), were collected. The Numeric Rating Scale (NRS) was used to assess the degree of pain in patients. Clinical indicators before and 6 months after treatment were compared between the two groups.ResultsThere was no statistically significant difference in age and gender between the two groups. After 6 months of treatment, the effective rate in the study group (48 cases, 97.96%) was higher than that in the control group (42 cases, 85.71%), with a statistically significant difference (p = .027). At the same time, the study group had significantly lower levels of serum uric acid (162.39 μmol/L ± 17.23 μmol/L vs. S198.32 μmol/L ± 18.34 μmol/L, p < .001), creatinine (87.39 mmol/L ± 9.76 mmol/L vs. 92.18 mmol/L ± 9.27 mmol/L, p = .014), total cholesterol (3.65 mmol/L ± 0.65 mmol/L vs. 4.76 mmol/L ± 0.73 mmol/L, p < .001), and triglycerides (1.76 mmol/L ± 0.32 mmol/L vs. 2.28 mmol/L ± 0.41 mmol/L, p < .001) compared to the control group, with statistically significant differences (p < .05). After treatment, the levels of inflammatory factors and degree of pain in the study group were significantly lower than those in the control group (all p < .05). During the treatment process, the incidence of adverse reactions in the study group (2 cases, 4.08%) was lower than that in the control group (9 cases, 18.37%), with a statistically significant difference (p = .025).ConclusionFebuxostat combined with a low‐purine diet can reduce inflammatory factors and alleviate the degree of pain in gout patients, significantly improving their clinical symptoms.

Gout arthritis adalah masalah kesehatan umum pada agregat lansia. Penderita gout arthritis di Seksi Kesejahteraan Sosial Padu Wau Waipare sebanyak 18 dari 65 lansia. Penderita gout yang mengalami inflamasi sendi, jika tidak mendapat terapi dapat mengakibatkan kecacatan fisik. Terapi non-farmakologis yang dapat dilakukan untuk menurunkan asam urat adalah edukasi diet rendah purin, rendah lemak dan mengatur aktivitas fisik tingkat sedang. Tindakan terapeutik pemberian kompres jahe serai hangat adalah salah satu intervensi keperawatan manajemen nyeri. Jahe mengandung gingerol limonene, acid aspartic sedangkan serai mengandung minyak astiri yang bermanfaat untuk penghangat dan anti radang. Kompres hangat dapat menstimulasi vasodilatasi memperlancar sirkulasi darah sehingga dapat mengurangi sensai nyeri. Studi kasus ini bertujuan untuk mengurangi nyeri dan menurunkan kadar asam urat lansia gout arthritis setelah dilakukan tindakan keperawatan terapi non-farmakologis. Penelitian ini menggunakan metode case study, responden 2 lansia dengan kriteria inklusi keluhan nyeri, tingkat aktivitas rendah <600MET, kadar asam urat darah >7mg/dl dan tidak rutin minum obat. Hasil studi kasus pada kedua subjek menunjukkan penurunan nyeri dan kadar asam urat setelah terapi selama 1 minggu. penurunan kadar asam urat subjek studi 1 sebesar 5,8 mg/dl sedangkan subjek studi 2 sebesar 3,5 mg/dl. Penurunan skala nyeri kedua subjek studi kasus sebesar 2 poin.
