Non-Operative Rehabilitation of Isolated Popliteus Tendon Rupture in a Rugby Player

Abstract: Isolated rupture of the popliteus tendon is uncommon; instead, it is often seen as part of multi-ligamentous posterolateral corner injuries. In this report, we present a case of a 22-year-old professional rugby player who sustained a lateral blow to his semi-flexed knee in a tackle during a competitive game. A complete popliteus tendon rupture at its musculo-tendinous junction was diagnosed on magnetic resonance imaging despite a relatively unremarkable physical examination. The aims of this report are to high… Show more

“…Guha et al 3 reported return to activity at 6 weeks and no instability at 12 months with non-operative management of a semi-professional athlete with complete intra-substance rupture. Koong et al 4 similarly described return to full contact activities at 4 weeks in a 22-year-old professional ruby player who had sustained a complete popliteus tendon rupture at the musculotendinous junction. This player returned to regular season matches soon after.…”

“…Popliteal integrity decreases the external rotation laxity seen in full-thickness popliteal tendon tears. However, there is also a role for conservative management in these injuries, 10,11 and future prospective studies are needed to determine the relative outcomes of surgical intervention as opposed to conservative management. In this case, a full-thickness popliteal injury was diagnosed preoperatively based on both clinical examination and imaging findings, and the surgical approach was already necessary to treat the meniscal pathology.…”

“…1,7 Furthermore, monosodium urate crystals that induce gout commonly collect in the popliteal hiatus and may also weaken the tendon, thereby leading to a susceptibility for tearing. 8,9 The management of isolated popliteal tendon injuries remains controversial, with some authors favoring conservative management 10,11 but others proposing surgical intervention for acute and chronic grade 3 injuries of the popliteus. 7,12 The rationale for acute intervention in popliteal tendon injury is to restore the biomechanical integrity of the posterolateral corner, thereby decreasing stress on and potential future injury to the FCL, popliteofibular ligament, lateral compartment, and cruciate ligaments.…”
