Open bite is the absence of vertical overlap of lower incisors by upper incisors when posterior teeth are in occlusion. An anterior open bite can cause periodontal damage which leads to gingivitis and periodontitis. To assess the gingival health status in patients with anterior open bite and also compares with patients without an anterior open bite. Retrospective data collected from 89,000 case records from June 2019 to March 2020. The present study consisted of 76 patients divided into two groups: patients with anterior open bite and patients without an anterior open bite. In both groups, the following parameters were recorded - Oral Hygiene Index - Simplified (OHI-S) and Plaque Index (Silness and Loe). OHI-S index and plaque index was similar in patients with and without an anterior open bite. Mean Plaque Index for case group (patients with anterior open bite) is 0.76, and Mean Plaque Index for control group (patients without the anterior open bite) is 0.78. Mean OHI-S Index for case group (patients with anterior open bite) is 0.51, and Mean OHI-S Index for the control group (patients without the anterior open bite) is 0.53. The difference was not statistically significant (P-value > 0.05). In this study, gingival health status in patients with and without an anterior open bite is low.

Introduction. Periodontal disease (PD) is a chronic inflammation of the soft tissues that support the structure of the tooth, and miRNAs are highly dynamic molecules that participate in the regulation of gene expression interfering with multiple genetic targets. The dysregulation of the expression of miRNAs has been associated with different types of pathologies; therefore, they are excellent molecules to be studied as biomarkers. Material and Methods. A search was made in the electronic databases of PubMed, Scopus, and Science Direct. The following key words were used: “microRNAs,” “miRNAs,” “periodontal disease,” “periodontitis,” and “biomarker”; employee independent search strategies with the Boolean operators “OR” and “AND”; a further search of the references of the selected studies was performed to detect potential studies that met the selection criteria. The data recollected from each article were author, country, year of publication, sample size, type of sample used to identify miRNAs, methodology used to identify miRNAs, type of periodontal disease, and miRNAs identified. Results. Of the 13 selected studies, 6 used gingival tissue as a sample for the identification of miRNAs, 3 used gingival fluid, 2 used saliva, 1 used serum, and another used periodontal tissue. Chronic periodontitis was the most studied periodontal disease in 9 of the 13 selected articles; 7 used microarrays as the main technique for the identification of miRNAs. qRT-PCR was the assay choice to validate the identified miRNAs. Conclusion. The main type of periodontal disease on which most studies are focused is chronic periodontitis, with the main miRNAs being hsa-miR-146a, hsa-miR-146b, hsa-miR-155, and hsa-miR-200. This systematic review is one of the first to carry out an analysis of the current role of miRNAs in PD as biomarkers.

Once a while, disease classifications have needed revision because new knowledge has accumulated, and new technologies and better treatments have emerged. Changes made to disease classifications should be trustworthy and openly justified. The periodontitis definition and classification system was changed in 2017 by the ‘World Workshop on the Classification of Periodontal and Peri‐Implant Diseases and Conditions’. The workshop, comprising clinicians and researchers, resulted in the production of a 23‐article special issue that introduced the new definitions and classifications of periodontitis. In this narrative review, we critically review how the changes made to the periodontitis definition and classification were justified in the light of the Checklist for Modifying Disease Definitions. Under each of the eight items of the checklist, we have discussed how the item was or could have been considered in the light of the checklist and its guidance. In our view, the new definition and classification of periodontitis was presented in an understandable way, even though the changes from the previous definition were not made visible. However, the issues of (1) estimated changes in prevalence or incidence, (2) triggers for the change, (3) prognostic ability, (4) repeatability or reproducibility, (5) incremental benefits, (6) incremental harms or (7) net benefits and harms related to the introduction of new classification were not considered in the way suggested in the checklist. Thereby, a balanced assessment of potential benefits and harms associated with the new periodontitis classification system was not presented, and to a large extent it remains unknown if the use of the new classification system will provide more net benefits to patients and to the community than previous systems. It is our view that patients and societies deserve transparent and balanced assessments of the potential benefits and harms associated with the periodontitis classification. Importantly, these should reflect the values and preferences also of the patients and the wider community and consider the impact on resource usage.
