Non‐Helicobacter pylori Related Duodenal Ulcer Disease in Children

Abstract: 'Idiopathic' (H. pylori-negative, NSAIDs-negative) duodenal/gastric ulcers are present in symptomatic children. Clinical or endoscopic characteristics are insufficient markers to identify those 'idiopathic' ulcers. Investigating the 'risk factors' for those ulcers will be helpful in reducing the morbidity in these children.

“…18 nevertheless, a much lower frequency (around 2%) of ulcers was reported in some other countries. 12,14,16 the male predominance, found in some studies, may be due to endocrinologic factors. 23,24 the apparent different prevalence of gastro-duodenal lesions between these different studies may be biased by the fact that the lifestyles are not comparable, unequal availability of endoscopy facilities, referral policy to third level hospitals with endoscopy facilities, total number of endoscopy per year, proportion of older children in each center (lesions more frequent in older ones), PPi's prescription policy without endoscopy or H. pylori frequency in the local population.…”

“…This figure is surprising, since we recently reported a series of duodenal ulcer cases with 95.3% of the patients with H pylori (5) . However, other ulcer cases have to be taken into account, such as use of anti-inflammatory drugs and non-H pylori related ulcer disease (3) . Change in the profile of patients who attend the service may be reflected in the found result.…”

“…Peptic ulcer usually develops in children secondary to a major stressful condition or use of NSAIDs or corticosteroids [39]. The limited knowledge on this issue implies that peptic ulcers are not rare even during childhood and that the frequency shows a wide variation in different geographical areas, ranging from 5-35% [40][41][42].…”
