Nomograms Provide Improved Accuracy for Predicting Survival after Radical Cystectomy

Abstract: Aims: To develop multivariate nomograms that determine the probabilities of all-cause and bladder cancer^specific survival after radical cystectomy and to compare their predictive accuracy to that of American Joint Committee on Cancer (AJCC) staging. Methods: We used Cox proportional hazards regression analyses to model variables of 731 consecutive patients treated with radical cystectomy and bilateral pelvic lymphadenectomy for bladder transitional cell carcinoma. Variables included age of patient, gender, pa… Show more

“…[26][27][28] Nonetheless, many previously reported models have shown an accuracy of < 70%. [29][30][31][32][33] Finally, no other models are capable of predicting cancerspecific-mortality-free rates after nephroureterectomy with or nephroureterectomy without bladder-cuff removal.…”

“…These limitations are shared with all studies that have resulted in the development and validation of predictive or other prognostic and predictive models. [7][8][9][10][11]24,[26][27][28][38][39][40][41][42] Possibly, the use of biomarkers could improve its accuracy. The ability of biomarker data has been confirmed to improve nomogram predictions.…”

“…We and others have previously shown that nomogram-based prediction models based on current histopathologic criteria can improve accuracy over AJCC-based stage groupings. [11][12][13][14] However, these models were not perfect and there is still a need to improve the predictive accuracy for the individual patient based on biologic information. Accurate prediction of response to therapy, disease recurrence, and survival may aid individual patient counseling, frequency and extent of monitoring, and adjuvant therapy planning.…”

“…Radical cystectomy is the gold standard in the treatment of MIBC and provides excellent local control of the disease with a better prognosis than not undergoing radical cystectomy (22,23). Thus, the prognosis is significantly different between cystec- Figure 2.…”
