La desnutrición hospitalaria es un problema ampliamente reconocido en el mundo, donde el paciente pierde nutrientes, complica la enfermedad y puede llegar a fallecer. Este estudio descriptivo realizó una revisión sistemática de 40 artículos científicos con el objetivo de exponer los factores de riesgo relacionados con la desnutrición hospitalaria y mortalidad. Se evidencio que la desnutrición hospitalaria está influenciada por factores biológicos, como la edad y características individuales, y por indicadores físicos como estatura baja, pérdida de peso y adiposidad. El contexto social y económico también juega un papel importante al dificultar el acceso a una alimentación adecuada. Además, la depresión y las condiciones emocionales adversas aumentan el riesgo de desnutrición. Es esencial abordar la nutrición durante la hospitalización para prevenir complicaciones y reducir costos asociados.

BACKGROUND: Cirrhosis is a chronic and progressive liver disease that occurs from prolonged hepatocellular injury. Malnutrition causes complications in cirrhosis patients that worsen the condition to liver failure. Both are closely linked and increase the chances of morbidity and mortality. Regular nutritional screening and monitoring is prime concern for such patients including comprehensive dietary history, laboratory tests, and evaluation of muscle loss and strength capabilities to determine the degree of frailty. For efficient assessment of liver cirrhosis patients Subjective Global Assessment has been used worldwide. The nutritional objectives for such individuals should be to regain liver functions, to prevent complications associated, and to overcome nutritional deficiencies causing malnutrition. METHODS: We conducted a literature review using PubMed, Google Scholar and Science Direct for this purpose, a total of 130 articles were reviewed out of which 80 (from the past 5 years) including originally published research, review articles and abstracts were also included. Exclusion criteria of the selected studies was year of publication, irrelevancy and animal studies based on the purpose of current study. The aim of this study was to check nutritional management in patients having complications of liver cirrhosis. RESULTS: According to the guidelines, for the conservation of normal nutritional status of the malnourished patients’, energy should be provided 35 kcal/kg/day while to prevent hypoalbuminemia and maintain the protein stores in the body, 1.5 g/kg/day protein has been recommended. Carbohydrates and fats for cirrhosis patients are recommended 50% to 60% and 10% to 20% of the total dietary intake respectively. CONCLUSION: Initial identification and prevention of malnutrition have the probability to lead to better health outcomes, prevention of complications of the disease, and improving quality of life.

Background: Malnutrition is associated with increased short and long-term mortality in patients with end-stage liver disease (ESLD). We launched this study to compare subjective global assessment (SGA) and nutrition risk index (NRI) as malnutrition screening tools in End-stage Liver Disease (ESLD) patients. Methods: This cross-sectional study was conducted on liver transplant patients in Montaserieh Hospital of Mashhad and Firoozgar Hospital of Tehran from May to September 2021. Nutritional status of the patients was categorized into normal nutritional status, moderate and severe malnutrition based on both the NRI and SGA. Sensitivity, specificity, and predictive values of NRI were evaluated compared with SGA in ESLD patients. Results: Forty-three cirrhotic patients who were candidates for liver transplantation were assessed. Mean age and BMI of the patients were 48±14 years and 24.5±5.4 kg/m², respectively. Based on SGA, the prevalence of normal nutrition status, moderate and severe malnutrition were 18.6%, 37.2%, and 44.2%, respectively. Based on NRI, the prevalence of normal nutrition status, mild to moderate, and severe malnutrition were 11.6%,42%, and 46.4%, respectively. The sensitivity, specificity, positive and negative predictive values for NRI in detecting malnutrition based on SGA were 94.2%, 37.5%, 86.8% and 60%, respectively. However, the agreement between NRI and SGA was low (k=0.284). Changing the NRI cut-off value to 87.2 could yield acceptable sensitivity (75%) and specificity (71.4%). Conclusion: NRI can be used as a screening tool in ESLD patients but a different cut-off might be required to improve its validity against SGA in ESLD patients.
