Nodular erythema elevatum diutinum mimicking Kaposi′s sarcoma in a human immunodeficiency virus infected patient

Abstract: Erythema elevatum diutinum (EED) has been emerging as a specific Human Immunodeficiency Virus (HIV) associated dermatosis in recent times. It is an extremely rare chronic disease of unknown origin and part of the spectrum of leukocytoclastic vasculitis. We describe a case of EED simulating Kaposi's sarcoma in a 52-year-old HIV infected female patient with no previous opportunistic infections and CD4+ count of 164/mm3. Therapy with oral dapsone (100 mg/day) for two weeks resulted in resolution of some lesions.

“…Surgical excision may be appropriate in the setting of late-stage, fibrotic lesions 19 . However, of the 10 late-stage lesions treated with dapsone with known response, 5 completely or partially resolved, 3 discontinued dapsone because of adverse events, and 2 did not respond to treatment 3 , 5, 6, 7 , 9 , 10 , 14 , 16 , 19 , 20 . This finding may indicate that surgical excision (or other therapy) is more appropriate after treatment failure with dapsone.…”

“…4 Two cases were not concomitant with an underlying disease, 1 was found associated with Behcet disease, 5 and 1 was associated with HIV and hepatitis C and B viral co-infections. 6 In the reported palmoplantar cases, ages ranged from 32 (HIV patient) to 77, with an average of 54 years. We are not able to explain why, in our case, the patient, at 83 years of age, was considerably older than the other patients studied.…”

“…The patient was initially treated with oral dapsone but was switched to colchicine as she developed hypersensitivity to dapsone. [13]…”
