Alterations in the volume of platelets (mean platelet volume, MPV; platelet distribution width, PDW) and erythrocytes (red blood cell distribution width, RDW) have been reported in rheumatoid arthritis (RA) and might serve as diagnostic biomarkers. We conducted a systematic review and meta-analysis of the MPV, PDW, and RDW in RA patients and healthy controls. Relevant articles were searched in PubMed, Web of Science, Scopus, and Google Scholar from inception to June 2022. Risk of bias was assessed using the Joanna Briggs Institute Critical Appraisal Checklist and certainty of evidence was assessed using GRADE. In 23 studies (2194 RA patients and 1565 healthy controls), the RDW, but not MPV or PDW, was significantly higher in RA patients (standardized mean difference, SMD = 0.96, 95% CI 0.78 to 1.15, p < 0.001; moderate certainty of evidence). The substantial heterogeneity observed (I2 = 75.1%, p < 0.001) was virtually removed in a subgroup of prospective studies. In sensitivity analysis, the magnitude of the effect size was not substantially modified by sequentially removing individual studies. There was no significant publication bias. No significant associations were observed between the effect size and pre-defined study or patient characteristics. The results of our study suggest that the RDW might be a useful biomarker for the diagnosis of RA, and complement the clinical information provided by other patient characteristics and laboratory parameters (PROSPERO registration number: CRD42022349432).

ÖzAmaç: Ankilozan spondilit (AS) hastalarında kardiyak tutulum %10-30 oranında görülür. Kronik inflamasyon, ankilozan spondilitte olduğu gibi kardiyovasküler hastalıklar, hipertansiyon ve diürnal kan basıncı değişkenliğinin patofizyolojinde rol almaktadır. Azalmış kan basıncı değişkenliği kardiyovasküler olaylar için yüksek risk göstergesidir. Bu çalışma ile amacımız AS hastalarında diürnal kan basıncı değişikliğinin normal popülasyonla karşılaştırılmasıdır.Gereç ve Yöntem: Bu çalışma için 25 AS hastası ve 32 kontrol alınmıştır. Hipertansif hastalar, hipertansif tedavi almakta olan veya ambulatuar kan basıncı monitorizasyonu (ABPM) sonuçlarına göre gündüz kan basıncı ortalaması SKB/DKB>135/85 mmHg ve/veya gece kan basıncı ortalaması SKB/DKB>120/70 mmHg olması şeklinde tanımlanmıştır. Dipper paterni gece kan basıncı ortalamasının gündüze oranla %10 ve daha fazla düşmesi olarak tanımlandı.Bulgular: Araştırmanın yaş ortalaması 42.5±12.1 yıldır ve % 63.2'si erkektir. Kontrol grubu ve AS grupları arasında temel demografik özellikler açısından anlamlı fark yoktur. ABPM sonuçlarına göre; AS grubunun %36'sı (n=9) ve kontrol grubunun %46.9'u (n=15) hipertansif olarak belirlenmiştir ve gruplar arasında istatistiksel fark saptanmamıştır (p=0.290). AS hastaları ve kontrol grubu arasında ortalama, gündüz ve gece; sistolik ve diyastolik kan basıncı sonuçları açısından anlamlı fark saptanmamıştır (p>0.05). AS hastalarının %76.0'ında (n=19) ve kontrol grubunun %71.8'inde (n=23) non-dipper patern saptanmıştır. Bu oranlar arasındaki fark istatistiksel açıdan anlamlı değildir (p=0.949). Gece gündüz arası sistolik kan basıncı değişkenlikleri açısından gruplar arasında fark saptanmamıştır (p=0.772). Sonuç:Çalışmamızın sonuçlarına göre non-dipper paterni ve diürnal kan basıncı değişimi AS hastalarında ve kontrol grubunda benzerdir. Hipertansif ve normotansif hastalar alt grup analizinde değerlendirildiğinde de gruplar arasında non-dipper hastaları ve diürnal kan basıncı değişkenliği açısından fark saptanmamıştır.

Introduction: Various physiological and psychological effects of earthquakes can be seen on the human body, even without a direct physical impact from the earthquake. Both the experience of the earthquake process and the earthquake-related subsequent life changes cause stress through the activation of the sympathetic and parasympathetic systems in the body. This stress can affect various physiological processes, including the cardiovascular system. In this review, the effect of earthquakes on the cardiovascular system was aimed to be discussed in line with the available evidence. Discussion: There are studies showing that natural events such as earthquakes increase the incidence of adverse cardiac events, such as myocardial infarction, heart failure, hypertension, and sudden cardiac death, or cause existing cardiac diseases to worsen during these periods. However, there are also some evidences with conflicting results. Therefore, the effect of earthquakes on cardiovascular diseases has not been clearly demonstrated yet. Conclusion: Even survivors of earthquakes without physical injury are exposed to stress secondary to both internal and external factors. Studies indicate that individuals who are involved in the earthquake process may have adverse effects on cardiovascular health in the short and long term. Therefore, it is important to take necessary precautions and create appropriate conditions, especially in people with cardiovascular disease.
