Infection due to Nocardia is reported mainly in immunocompromised patients. It usually presents as a pulmonary or disseminated disease with a predilection for the brain. Infections are a rare etiology of intracranial vascular aneurysms. Herein we report a case of disseminated Nocardia otitidiscaviarum ( N . otitidiscaviarum ) in a young female newly diagnosed with systemic lupus erythematosus (SLE) complicated by the development of an infectious intracranial aneurysm. To the best of our knowledge this is the fourth case of nocardial infection-related intracranial aneurysm and the second case of N. otitidiscaviarum infection to be reported in a patient with systemic lupus erythematosus. Features of previously reported N. otitidiscaviarum related intracranial aneurysm are reviewed.

This chapter starts by considering the resident bacterial population of the skin and how this has altered with our growing understanding of the skin microbiome. It then reviews the whole range of bacterial skin infections from primary Gram‐positive infections due to staphylococci and streptococci to the less common infections caused by Gram‐negative and anaerobic bacteria. The cutaneous manifestations of common and rare systemic bacterial infections such as staphylococcal septicaemia and melioidosis are described in practical detail. Using new work on antibiotics, the spread of drug resistance amongst skin bacterial pathogens is reviewed and its relevance clarified in the treatment algorithms. Rickettsial infections are also considered.

The skin provides a barrier between the body and the external environment and it is colonised by the bacteria and other microorganisms that make up the skin microbiome. Bacterial infections occur when bacteria invade through damage to the skin barrier, via trauma or pre‐existing skin conditions, or through inoculation or inhalation; bacterial skin infections can range from minor irritations to life‐threatening conditions. This chapter explores bacterial infections which can affect the skin, from the common to the rare, describing the epidemiology, presentation and management of each condition.
