No difference in the incidences of vasculitides between north and south Germany: first results of the German vasculitis register

Abstract: First results from a population-based vasculitis register serving nearly 5,000,000 inhabitants in north and south Germany revealed no regional differences in the incidence of all PSVs between north and south. The incidence rates of ANCA-associated PSVs, such as WG and MPA, were lower than those in the UK and Norway but higher than that in Spain.

“…However, CSS is a rare pathology, multisystemic, seldom taken care of exclusively in outpatient [8]. These results are also superimposable with other important European series (incidence: 1-4/million inhabitants/ annum within the general population [9][10][11][12] and prevalence: 2-13/million inhabitants [12][13][14]), confirming the absence of North-South or East-West gradient within this pathology [13] or evolution in time [11] contrary to other vasculitis as the disease of Wegener whose incidence is currently in increase and who has a North-South decreasing gradient [15].…”

“…A previous investigation in cooperation with a similar structured region in Baden-Wü rttemberg over a period of 2 years (University of Freiburg) revealed almost identical incidence rates for both regions in 1998 and 1999 (17). Because 2 years is too (Table 3).…”

“…In a population-based epidemiologic study covering an area of 5 million inhabitants, all patients newly diagnosed with PSV according to the Chapel Hill Consensus Conference (CHCC) were included in the register (16). No difference of the incidences and the entities between northern and southern Germany were observed within the first 2 years (17). Because of the relatively short observation period to obtain reliable population-based data, the vasculitis register was continued in Schleswig-Holstein (population 2.78 million) and the results from 1998 until 2002 are presented in this article.…”

“…Increased mortality in cases ascertained from HES compared to CPRD has been found in other conditions, [20] which shows the importance of ascertaining cases from secondary care data in addition to GP records, as patients ascertained in hospital are more likely to be sicker. The lowest reported 1-year mortality of 1-7% comes from registry studies, or highly selective (tertiary-referral) centres, [21,22] where patients also have the lowest average age. An intermediate level of 5-11% is found in community studies.…”
