Background: Cutaneous leiomyoma (CL) is a benign smooth muscle tumour included in painful skin tumours. Multiple CLs are cutaneous markers of hereditary leiomyomatosis and renal cell cancer (HLRCC). Objectives: To retrospectively review our series of patients with CLs to analyse their clinical features and the association with HLRCC. Methods: Cases coded as CL in the database of the pathology department between 2004 and 2019 were included in the study. Medical records were retrospectively reviewed to obtain the following data: age, sex, location, number of lesions, diameter, evolution time at diagnosis, suspected clinical diagnosis, tenderness, status of resection margins, development of recurrence, follow-up time, and association with HLRCC. Results: 152 patients had CLs, 89 women and 63 men, mean age 56.26, SD 16.030 years. Subtypes were piloleiomyoma in 62 patients, angioleiomyoma in 80, and genital leiomyoma in 10. All of our 11 patients with multiple lesions corresponded to piloleiomyomas, and HLRCC was confirmed in 8 of them (73%). Patients with HLRCC were younger than patients with piloleiomyomas without HLRCC (34.88 vs. 56.17 years, p = 0.009). Vascular and genital leiomyomyomas were solitary and were not associated with HLRCC. Conclusion: In patients with multiple piloleiomyomas HLRCC must be ruled out as it is confirmed in a high proportion of cases. The probability of fumarate hydratase mutation is greater in multiple piloleiomyomas involving both the trunk and upper extremities in the same patient.

Background/Aim: Recently, we reported a myoid hamartoma carrying a t(5;12)(p13;q14) karyotypic aberration leading to fusion of the high-mobility group AT-hook 2 (HMGA2) gene with a sequence from chromosome sub-band 5p13.2. We describe here another benign myoid tumor of the breast with identical genetic aberrations. Materials and Methods: A mammary leiomyomatous tumor found in a 45year-old woman was studied using cytogenetics, fluorescence in situ hybridization, RNA sequencing, reverse transcriptionpolymerase chain reaction and Sanger sequencing. Results: The karyotype of the tumor cells was 46,XX,t(5;12) (p13;q14) [14]. Fluorescence in situ hybridization showed rearrangement of HMGA2, RNA sequencing detected fusion of HMGA2 with a sequence from 5p13.2, whereupon reverse transcription-polymerase chain reaction together with Sanger sequencing verified the HMGA2-fusion transcript. The results were identical to those obtained by us previously in a myoid hamartoma of the breast. Conclusion: The translocation t(5;12)(p13;q14) and fusion of HMGA2 with sequences from sub-band 5p13.2 appear to be recurrent events in benign mammary myoid neoplasms.

Fundamento. Los leiomiomas cutáneos, superficiales o suprafasciales, se dividen en tres variantes: piloleiomiomas (PL), angioleiomiomas (AL) y leiomiomas genitales (LG) que incluyen las formas vulvares, escrotales y areolares. El objetivo fue establecer las características clinicohistológicas y la incidencia de cada variante, y las posibles asociaciones con neoplasias internas. Material y métodos.Se revisaron 255 casos de leiomiomas cutáneos diagnosticados entre 1982 y 2018 en los servicios de Anatomía Patológica de tres centros hospitalarios (Navarra y Alicante). Se describieron y compararon variables demográficas, clínicas, histológicas e inmunohistoquímicas.Resultados. La incidencia en Navarra de PL fue 4,3 casos por año y millón de habitantes, de AL 20 y de LG 1,4. Las formas cutáneas suponen aproximadamente el 3,5 % del total de leiomiomas. La población con PL sufrió más frecuentemente cáncer de mama (OR = 4,8; IC95%: 1,3-17,4; p = 0,006). Los leiomiomas areolares son de pequeño tamaño, acompañados de dolor local, predominantemente fasciculares o sólidos, con muy rara afectación del tejido celular subcutáneo y escasa atipia. Esto contrasta con el resto de LG, de tamaño medio y rara vez dolorosos, predominantemente nodulares, con frecuente afectación del tejido celular subcutáneo y atipia. Conclusiones.Se aporta información sobre las características clinicohistológicas de las distintas variantes de leiomiomas, según las cuales debería replantearse la clasificación de los leiomiomas areolares fuera del grupo de LG. Se detectó una asociación entre PL y carcinoma de mama que deberá confirmarse en futuros estudios para determinar si este leiomioma constituye un marcador de riesgo de cáncer de mama en mujeres.
