Introducción: El trabajo por turnos tiene el potencial de perturbar la vida familiar y social, y puede provocar fatiga crónica, somnolencia y síntomas somáticos asociados a trastornos del ritmo circadiano. Objetivo: Comparar la calidad de vida, estrés laboral e indicadores de salud en profesionales de enfermería que trabajan con turnos rotativos y diurnos. Materiales y métodos: Estudio descriptivo de diseño observacional y de corte transversal con 24 participantes divididos en 2 grupos de acuerdo con el tipo de sistema de trabajo que desempeñaban: diurno y turnos rotativos. Se aplicaron los cuestionarios SF-36 y Burnout, y se midieron algunos indicadores de salud. Resultados: Las/os enfermeras/os que trabajan en turnos rotativos presentaron una calidad de vida más baja en la dimensión rol emocional (p=0,039), mayor cansancio emocional (p=0,046), menor sentimiento de realización personal en el trabajo (p=0,022), menores niveles de insulina basal (p=0,005), menor valor en el índice de HOMA-IR (p=0,007) y mayor cantidad de METs (Metabolic-energy-equivalents) gastados semanalmente (p=0,041) en comparación con sus pares que se desempeñan en jornadas diurnas. Conclusiones: Las/os enfermeras/os que trabajan en turnos rotativos presentan una disminución de la calidad de vida, mayores niveles de actividad física y estrés laboral que quienes trabajan de forma diurna.

In organizing health services in hospitals, the nursing team is the frontline for achieving service for 24 hours continuously, therefore in some division, like ICU or ER, nurses work with shift hours. There is some evidence of shift work’s psychological and physiological effects that can influence nurses' quality of life. This study aims to obtain an overview of the quality of life in nurses who work into the shift and its comparison with non-shift. This study was an online survey that involving nurses from hospital-based in Bandung. The survey was distributed online, where participants who participated in the survey had certain criteria. Data collection carried out with questionnaires from WHOQOL-BREF, designed using a cross-sectional comparative study model. Forty-five nurses were included in this study with 82% are woman, with 77,7% work with shift and 22,2% no-shift. Overall, the quality of life of nurses works in shift decreased in the physical domain. Specifically, those who perceived themselves had physical pain. Even so, they still perceived good quality of sleep. Nurses work in shift also perceived negative emotion such as feeling blue, anxiety and depression. There were two differences between nurse with shift work and non-shift work in quality of life domains (physical, U= 83,5, p-value= 0,012) and (environment, U= 102,5, p-value= 0,047). Physical pain, less free time to relax, and physical activity such as exercising may influence the lower quality of life. Nurses with shift work and nurses with non-shift work also found significantly different in their overall health (U=84, p-value=0,008). It might be related to how they perceived their health issue.

Aim and objectives To explore the perceptions and experiences of Cambodian ICU registered nurses regarding their working 24‐hr shifts. Background In Europe and the USA, nurses are moving to a 12‐hr shift, and numerous studies have revealed the positive and negative effects of these. However, lesser known is the impact of 24‐hr nursing shifts on care quality, and health and safety. In Cambodia, 100% of nurses are rostered for these in their shift patterns, but until this study no research had been conducted on such shifts. Design A qualitative descriptive study. Method Three focus group discussions were conducted with 30 registered nurses in July 2019, ten in each group, from three intensive care units of three hospitals in Cambodia. Data saturation was obtained. Data were analysed using content analysis, and the COREQ was applied for reporting this study. Findings The ICU nurses’ perspectives revealed significant and unacceptable effects of working shifts of ~25‐hr length, taking into account staff handover. Two major themes arose: It is so exhausting and Compromised hospital care. Alarmingly, participants worked on average 72 hr per week, were exhausted, and nursed between 6 and 10 critically ill patients per shift. Conclusion To our knowledge this is the first study on nurses working 24‐hr shifts, revealing unacceptable, high risks for the health and safety of nurses and patients, with nursing activities left undone, and a lack of quality care. Relevance to clinical practice Improving nurse and patient health and safety, and quality of care requires hospital leaders to work with government and nursing organisations to develop better shift strategies. Resources need to be provided so that: nurses can work a maximum 12‐hr shifts; the ratio of nurses to patients is improved; and nurses can have decent break times. This has major implications, for not only practice, but also management, administration, budgets and education.
