BACKGROUND Gastroesophageal reflux disease (GERD) is a common complication of esophageal cancer surgery that can affect quality of life and increase the risk of esophageal stricture and anastomotic leakage. Wendan Decoction (WDD) is a traditional Chinese herbal formula used to treat various gastrointestinal disorders, such as gastritis, functional dyspepsia, and irritable bowel syndrome. Mosapride, a prokinetic agent, functions as a selective 5-hydroxytryptamine 4 agonist, enhancing gastrointestinal motility. AIM To evaluate the therapeutic effects of WDD combined with mosapride on GERD after esophageal cancer surgery. METHODS Eighty patients with GERD were randomly divided into treatment (receiving WDD combined with mosapride) and control (receiving mosapride alone) groups. The treatment was conducted from January 2021 to January 2023. The primary outcome was improved GERD symptoms as measured using the reflux disease questionnaire (RDQ). The secondary outcomes were improved esophageal motility (measured using esophageal manometry), gastric emptying (measured using gastric scintigraphy), and quality of life [measured via the Short Form-36 (SF-36) Health Survey]. RESULTS The treatment group showed a notably reduced RDQ score and improved esophageal motility parameters, such as lower esophageal sphincter pressure, peristaltic amplitude, and peristaltic velocity compared to the control group. The treatment group showed significantly higher gastric emptying rates and SF-36 scores (in both physical and mental domains) compared to the control group. No serious adverse effects were observed in either group. CONCLUSION WDD combined with mosapride is an effective and safe therapy for GERD after esophageal cancer surgery. It can improve GERD symptoms, esophageal motility, gastric emptying, and the quality of life of patients. Further studies with larger sample sizes and longer follow-up periods are required to confirm these findings.

Latar belakang: Gastroesophageal Reflux Disease (GERD) adalah suatu gangguan dimana isi lambung mengalami refluks secara berulang ke dalam esofagus yang menyebabkan gejala dan/atau komplikasi yang mengganggu kualitas hidup. Pengobatan medis yang ada belum dapat memberikan hasil pengobatan maksimal. Tujuan: Untuk mengetahui manfaat kombinasi akupunktur dan herbal Tiongkok dalam menangani GERD. Metode: Penelitian quasi experimental menggunakan rancangan two-group pra-post test design. Jumlah subjek penelitian sebanyak 26 responden yang sudah dikelompokkan menjadi dua kelompok yaitu kelompok Akupunktur (13 orang) dan Kelompok Kombinasi Akupunktur dan Herbal (13 orang) menurut kriteria inklusi dan eksklusi. Pada kelompok Akupunktur diberikan Akupunktur pada titik ST 36 Zusanli, CV 12 Zhongwan, PC 6 Neiguan sebanyak dua kali seminggu selama sebulan, sedangkan pada kelompok Kombinasi Akupunktur dan Herbal diberikan Akupunktur pada titik ST 36 Zusanli, CV 12 Zhongwan, PC 6 Neiguan sebanyak dua kali seminggu dan Si Ni San @3 gr sehari dua kali selama sebulan. Hasil: Pada Kelompok Akupunktur dan Herbal, penggunaan titik akupunktur ST 36 Zusanli, CV 12 Zhongwan dan PC 6 Neiguan selama sebulan lebih efektif menurunkan skor GERDQ dengan rentang rata-rata perubahan skor GERDQ sebanyak 6.08 dengan p value 0,001. Kesimpulan: Penelitian menggunakan metode Akupunktur dan Herbal lebih efektif dibandingkan pemberian Akupunktur saja dalam membantu menurunkan skor GERDQ dan mengurangi gejala GERD.

Introduction: Refractory gastroesophageal reflux disease (rGERD) therapy is always lack of effective treatment in recent medical procedures. The potassium-competitive acid blocker (PCAB) vonoprazan has greater potency of acid inhibition than PPIs. The Chinese herbal medicine (CHM) Shugan-Hewei Decoction (SHD) has also demonstrated its efficacy in relieving symptoms and improving quality of life, but data on its efficacy combined with vonoprazan for rGERD are limited. This trial aims to evaluate the efficacy and safety of SHD combined with vonoprazan in the treatment of rGERD. Methods and analysis: This is a pilot, randomized, parallel-arm, placebo-controlled, multi-center clinical trial. A total of 124 participants will be allocated to the SHD group and control group randomly. The SHD group will receive SHD granules with vonoprazan (20mg/day). The control group will receive vonoprazan (20mg/day) plus an SHD granule placebo. Each group will be treated for 8 weeks. The primary outcome of the study is the score of the gastroesophageal reflux disease health-related quality of life questionnaire (GERD-Q). Secondary outcomes include healing rate, Acid exposure time (AET), oesophageal high-resolution manometry (HRM), frequency scale for the symptoms of GERD (FSSG), evaluation criteria of TCM syndrome efficacy, SF-36 quality of life scale (SF-36) and somatization symptom checklist (SSS). This trial aims to investigate the efficacy and safety of SHD combined with vonoprazan in the treatment of rGERD. Ethics and dissemination: This study has been approved by the Institutional Review Board of Human Research of Yueyang Hospital (approval number:2022-050). We will report the results of this study in a peer-reviewed journal and academic conference.
