New standardization of checking the vesicourethral anastomosis for tightness following radical prostatectomy with dynamic transrectal ultrasound: can this new technique replace the traditional postoperative cystogram?

Abstract: Following radical prostatectomy, the urine color is essential to assess the chance of a persisting leakage at the anastomosis. The method of dTRUS can replace the cystogram in checking for complete healing of the anastomosis, thus decreasing the exposure to X-rays of the patient of about 60 cGy/cm(2).

“…Eggert et al [10] reported a sensitivity of 100% and specificity of 97.5% using a TRUS for assessing the urinary leakage at the VUA site on postoperative day 6 in a series of 100 men undergoing RRP. Schenck and Schneider [11] developed a new method for evaluating the VUA using a dynamic TRUS with bladder filled only with NaCl 0.9% on postoperative days 4-8. If the urine is still cloudy or bloody, dynamic TRUS would be too early because a persisting VE at the anastomosis is very likely (96.9% likelihood).…”

“…Transrectal ultrasonography (TRUS) has been proposed as an alternative tool for detecting urinary leakage at the site of VUA and it can be performed directly by the urologist in the office with no need for transferring the patient to a radiology suite [10,11]. More recently, transrectal contrast-enhanced ultrasonography (CEUS) has been shown to be another reliable tool in this setting by offering a similar performance to CG [12].…”

“…The postoperative factors which increase the anastomotic leak are postoperative urinary tract infection [15] and pelvic para-anastomotic haematoma [15]. In addition, a bloody urine colour is a predictive factor for urethrovesical anastomotic leakage [16][17].…”

“…Urine color is one of the important attributes of urinalysis, which is shown to be useful in differentiating various medical situations. 2 In ancient medical practices, as suggested by the retained manuscripts, urine examination and specifically urine color has been frequently reported as a crucial diagnostic material for a wide variety of illnesses. 3,4 An example of such ancient medical practice is traditional Persian medicine, which, despite its long history, is considered today as a new brand of complementary and alternative medicine.…”
