New Paradigm in Explaining Dry Socket: Case Studies

Abstract: Dry socket is considered as the most common complication following tooth extraction. It is mainly manifested by severe irradiating pain that starts 2 to 4 days after dental extraction with a denuded socket. Although many risk factors are associated with it, no clear and direct cause has been described to explain its etiopathogenesis. Here, we suggest a new pathogenesis and a new management of  dry Socket based on new microbiological findings from 3 clinical cases. We present a series of 3 Dry Socket case… Show more

“…Based on a previous research, which involved positive bacterial cultures in four swabs out of six (P. aeruginosa and E. faecalis) [11], and the present study's findings showing a positive response in more than 86% of dry socket cases treated by ciprofloxacin, one can reach a conclusion that the pathogenesis of dry sockets cannot be solely attributed to a disorder in blood clot formation. The authors hypothesized that dry socket is a complication that may be caused by a specific bacterial infection, and the disorder in blood clot formation is considered a stage in the pathophysiological infectious process.…”

“…While the two patients who had negative responses to ciprofloxacin were out of reach for follow-up, their experience can be attributed to either a misdiagnosis of their case or bacteria resistant to ciprofloxacin. These bacteria might be Pseudomonas aeruginosa , Enterococcus faecalis , or other bacteria having similar properties [ 10 , 11 ]. The positive response to ciprofloxacin supports the hypothesis of an infectious process behind the dry socket.…”

“…Based on previous findings of bacterial culture and antibiogram on 3 reported cases of dry socket [9], this paper presents a serial case of dry socket of 6 patients aged 18 to 65 years old. For these patients, a topical treatment based on socket irrigation with saline followed by topical sedative dressing of eugenol was inefficient.…”

“…Pseudomonas aeruginosa appeared in the bacterial culture was sensitive to ciprofloxacin antibiotic [9]. However, due to its broad-spectrum activity, the efficacy of ciprofloxacin is not limited to Pseudomonas aeruginosa, therefore, other species can be suspected in the pathogenesis of dry socket.…”
