New Onset Migraine With Aura Following Head Injury: A Case Report

Abstract: A patient is described who developed recurrent headaches following a traumatic injury in the vicinity of the right greater occipital nerve (GON) that fulfill IHS criteria for migraine with aura. Both aura and headache can be triggered by pressure over the GON and relieved by local anesthetic blockade.

“…This case describes BTM aborted with GON blockade given because of prominent ipsilateral suboccipital tenderness noted on exam. GON blockade can alleviate migraine headache as well as aura 12 . Cessation of HPM auras with GON blockade was also reported in 2 patients 13 with rapid response very similar to the response we witnessed in our patient.…”

“…Cilostazol suppresses the expression of heparinbinding epidermal growth factor-like growth factor in lipopolysaccharide (LPS)-stimulated smooth muscle cells and macrophages [Kayanoki et al, 1997] and reduces superoxide anion formation and nuclear factor-kB (NF-kB) activation in high cholesterol-fed mice . Cilostazol also suppresses hyperplasia formation in balloon-injured rat arteries , prevents restenosis after coronary stent implantation by inhibiting Mac-1-leukocytes [Husid, 2004], and inhibits platelet-derived growth factor-stimulated cell proliferation by increasing cAMP levels and inhibiting the mitogen-activated protein kinase (MAPK) signaling pathway [Matousovic et al, 1995]. Because PDE3 activity and expression increase significantly in the aorta of atherosclerosis-prone insulin-resistant rats [Thomas et al, 1990], inhibition of PDE3 may be therapeutically useful for treating and preventing atherosclerosis.…”

“…Glutamate is a stimulator of CSD in laboratory studies 9 . Greater occipital nerve blockade (GON) can alleviate the pain of migraine and in 1 case report the migraine aura 10 . The results of GON blockade on the hemiplegic migraine aura and prolonged auras has not yet been studied.…”
