Dacryoliths consist partly of secreted mucins comparable with the mucin spectrum of the epithelium of healthy nasolacrimal ducts. Beside TFF1 and TFF3, both of which are produced under healthy circumstances, TFF2 is additionally induced and secreted in cases of dacryolithiasis. All three TFF peptides appear to be augmented in dacryoliths. With regard to their rheologic properties, TFF peptides may play a functional role in dacryolith formation. However, our results raise the question of whether TFF peptides per se influence dacryolith formation or whether their secretion, as in secretion of mucins and alpha defensins 1-3, is merely a secondary phenomenon.

Ophthalmologists and interventional radiologists are not the only professionals for whom diseases of the efferent tear duct system occupy centre stage; this applies also to ENT specialists involving endonasal conservative or surgical treatment. On the basis of current knowledge and taking account of results yielded by own research in recent years and of clinical aspects, we here give an overview of basic knowledge on the anatomy and physiology of the nasolacrimal system. In doing so functional aspects regarding tear transport as well as embryological and pathophysiological issues are integrated.

Objetivos: Epífora em pacientes nos quais a dacriocistografia mostra vias lacrimais permeáveis constitui ainda desafio ao especialista, especialmente quando não há lacrimejamento reflexo, nem alterações oculares externas, palpebrais ou de pontos lacrimais. Nestes pacientes, com provável obstrução incompleta, quisemos verificar o resultado da dacriocistorrinostomia endonasal endoscópica. Métodos: Doze pacientes com epífora e vias lacrimais permeáveis à dacriocistografia foram submetidos a dacriocistorrinostomia endonasal, utilizando-se a uncinectomia no meato médio. Resultados: Sete pacientes ficaram livres da epífora e três obtiveram melhora significativa após seguimento de 3 meses. Conclusão: A dacriocistorrinostomia endonasal é alternativa de tratamento válida para o tratamento de epífora em pacientes com vias lacrimais pérvias à dacriocistografia e sem alterações palpebrais.
