New aspects of the clinicopathological features and treatment of mycosis fungoides and Sézary syndrome

Abstract: Mycosis fungoides and S ezary syndrome are T-helper (Th)2-skewed cutaneous lymphomas. The clinical course of mycosis fungoides is classically indolent, manifesting as patches, plaques and tumors. Along with their progression, Th2 dominance tends to be accelerated. In this review, we discuss the epidemiology, clinicopathogenetic features and therapeutic approaches in mycosis fungoides and S ezary syndrome.

“…Cutaneous T-cell lymphoma (CTCL) are a heterogeneous group of non-Hodgkin's lymphoma that primarily affects the skin [103]. Mycosis fungoides (MF) is the most common CTCL type, representing 50-73% of cases, with a prolonged clinical course that progresses over years through patch, plaque, and tumor stages [104]. Sézary syndrome (SS) is a leukemic variant of CTCL with a poor prognosis.…”

“…Additionally, IL-25 is highly expressed in CTCL and promotes the production of Th2 cytokines, in particular IL-13, and eosinophil infiltration. Moreover, TSLP activates dendritic cells to induce Th2-mediated inflammation, and periostin induces chronic inflammation by stimulating TSLP production in CTCL [104,106,107]. This polarized Th2 cytokine pattern is less prominent in the early stages of MF, when Th1-derived IFN-γ is frequently detected, and emerges more markedly with time, as the Th2-bias of malignant T cells [106,107].…”

“…Because of the referred worsening of cutaneous clinical manifestations, we decided to perform new biopsies of skin lesions and immunohistochemical analysis revealed an epidermotropic lymphocytic infiltrate. The histological and immunohistochemical procedures were carried out according to the previously described methods ( 4 , 7 , 8 ). The immunohistochemical analyses, performed by using the antibodies listed in Table I , revealed a pattern with simultaneous presence of CD3 + , CD4 + and CD7 − antigens, confirming the diagnosis of MF ( Fig.…”
