New Application of 18F-Fluoride PET for the Detection of Bone Remodeling in Early-Stage Osteoarthritis of the Hip

Abstract: 18F-fluoride PET has great potential as an imaging method for diagnosing early-stage OA without any marked radiographic changes.

“…However, PET provides an alternative means that can elucidate molecular and cellular changes of the affected joints. Because changes in bone metabolism are present long before clear morphological signs, NaF-as well as FDG-PET can sooner identify and characterize early-stage OA, allowing for quicker treatment and closer monitoring of disease progression [58,59]. Furthermore, NaF demonstrates higher accuracy and sensitivity than 99m Tc-MDP and can reliably identify areas of high mechanical stress [60,61].…”

“…High 18 F-NaF uptake has often been observed in degenerative and arthritic knees during evaluation of osseous metastatic disease. 18 F-NaF PET may also be useful in the detection of bone remodeling in early stage OA of the temporomandibular and hip joints (17,66). SUV max was significantly higher in hip joints with an abnormal finding in bone on MRI (67).…”

“…F-NaF uptake on PET images are interpreted as processes that increase exposure of the surface of bone and provide a higher availability of binding sites, such as osteolytic and osteoblastic processes (17).…”

“…Kobayashi et al have demonstrated that an increased uptake of 18 F-Fluoride as a bone-imaging tracer represents early abnormalities in the subchondral bone, found prior to radiographic JSN [91]. The same group has further compared the diagnostic value of PET with MRI and radiography in early hip OA, and found that most (96%) of the MRI-positive joints were also PET positive (Fig.…”
