New Antipsychotics: Classification, Efficacy, and Adverse Effects

Abstract: Compared to traditional neuroleptics, most of the new antipsychotics are characterized by a low extrapyramidal side effect (EPS) liability and varying antipsychotic efficacy. This topic is reviewed for four principal classes of new, established, and potential antipsychotics: (1) Antipsychotics such as sulpiride and remoxipride that block a subgroup of dopamine (DA) D2/D3 receptors produce a relatively low level of side effects, including EPS, and have an antipsychotic effect equal to or slightly weaker than tr… Show more

“…The nigrostriatal dopaminergic neurons and mesolimbic dopaminergic neu rons are thought to be involved in the expression of the ex trapyramidal side effects and therapeutic effects of anti psychotics, respectively (9,44). Risperidone is useful for the treatment of schizophrenia because it has lower ex trapyramidal side effects than other antipsychotics in hu mans (19) Therefore, it is possible that the differences of the present biochemical findings between risperidone and haloperidol may indicate that risperidone has clinical advantages over haloperidol because it has less extra pyramidal side effects.…”

“…10,15 However, at higher doses, risperidone produces EPS, indicating that 5HT2A receptor antagonism alone cannot completely eliminate EPS associated with high D2 receptor blockade. The potential role of 5HT2A receptor antagonism in therapeutic responses to atypical antipsychotic drugs may become more apparent when data from clinical trials are available for the selective 5HT2A antagonists M100907.…”

“…Most atypical drugs are antagonists at D 2 receptors, yet often have other pharmacological properties, including concomitant D 1 , serotonin 5-HT 2 and ␣ 1 -adrenergic receptor blockade, or selectivity for other D 2 -like subtypes (Gerlach 1991). Much recent effort has been focused on producing compounds with selectivity for dopamine D 3 or D 4 receptors rather than D 2 receptors (Hartman and Civelli 1996).…”
