Newborn skin nodules are usually benign and self-resolving skin condition. Differential diagnosis of such lesions include cysts, hemangioma, abscess, cellulitis, sclerema neonatorum, subcutaneous fat necrosis, neurofibromatosis, benign tumors, or malignant tumors such as rhabdomyosarcoma, infantile fibrosarcoma, or neuroblastoma. We report a case of congenital subcutaneous fat necrosis in a 7-day-old baby presenting with multiple erythematous mass on back.

Panniculitides form a heterogenous group of inflammatory diseases that involve the subcutaneous adipose tissue. These disorders are rare in children and have many aetiologies. As in adults, the panniculitis can be the primary process in a systemic disorder or a secondary process that results from infection, trauma or exposure to medication. Some types of panniculitis are seen more commonly or exclusively in children, and several new entities have been described in recent years. Most types of panniculitis have the same clinical presentation (regardless of the aetiology), with tender, erythematous subcutaneous nodules. Although the patient’s age and the lesion site provide information, a histopathological assessment is sometimes required for a definitive diagnosis and classification of the disorder. In children, most panniculitides are lobular. At present, autoimmune inflammatory diseases and primary immunodeficiencies have been better characterised; panniculitis can be the presenting symptom in some of these settings. Unexplained panniculitis in a young child should prompt a detailed screen for monogenic immune disorders because the latter usually manifest themselves early in life. Here, we review forms of panniculitis that occur primarily in children, with a focus on newly described entities.

ABSTRAKNekrosis lemak subkutan pada bayi lelaki baru lahir merupakan panikulitis yang jarang terjadi. Kelainan ini dijumpai pada bayi baru lahir (BBL) cukup bulan atau lebih bulan, ditandai dengan nodus atau plak subkutan di area kulit yang terbatas/terlokalisasi. Kutis marmorata fisiologis ditandai oleh "patch" eritematosa yang tersusun retikuler dan membentuk gambaran seperti marmer. Lesi kulit ini merupakan respons vaskular tubuh bayi terhadap suhu dingin. Kondisi ini hilang timbul sesuai perubahan suhu lingkungan. Pada makalah ini dilaporkan satu kasus nekrosis lemak subkutan pada BBL disertai dengan kutis marmorata fisiologis. Diagnosis ditegakkan berdasarkan pemeriksaan klinis dan histopatologis. Faktor risiko yang ditemukan berupa riwayat asfiksia saat lahir dan riwayat preeklamsi berat pada ibu selama hamil. Tujuan laporan kasus ini adalah untuk meningkatkan kewaspadaan klinisi terhadap kelainan kulit yang mungkin dijumpai pada bayi baru lahir dan diharapkan dapat meningkatkan kemampuan dalam mendiagnosis dan mengelola kasus nekrosis lemak subkutan. It is characterized by subcutaneous noduses or indurated plaques on a localized skin. Physiological cutis marmorata is characterized by erythematous patch arranged in reticular form, resembling marble appearance, which is emanated as physiological vascular responsse to the temperature of the surrounding environment. We reported a case of SFN presenting with physiological cutaneous marmorata in a 22-day-old neonates who had history of neonatal asphyxia and related to maternal preeclampsia. By comprehending the clinical characteristic and management of this condition, clinicians should be aware to this condition that can be developed following various risk factors attributed to SFN and the complications that probably occurred.
