Neuropsychological rehabilitation in a patient with ruptured anterior communicating artery aneurysm: 48 month outcomes

Abstract: The neurobehavioral impairments associated with aneurysms of the anterior communicating artery (ACoA) are severe amnesia, executive problems and personality changes. Although most patients achieve a favorable neurological outcome, those cognitive deficits usually prevent return to previous activities and levels of social integration.ObjectivesTo report the outcomes of a neuropsychological and behavioral intervention in a 55 year-old man with very severe memory and executive dysfunctions following ACoA aneurysm… Show more

“…As portrayed in the case of Mrs. B, improvements in this population may take months and even years. This has been noted before by the few existing case studies focusing on the rehabilitation of this population (Deluca and Locker, 1996;Stablum et al, 2000;Bolognani et al, 2007). Anosognosia and memory impairments tend to compromise the process by which survivors learn about what happened to them and how impairments affect them in everyday life.…”

“…ACoA aneurysms are often associated with adverse cognitive outcomes, particularly executive and memory dysfunction, presumably because of their anatomic location. 43 This discrepancy may bias neurocognitive outcome measures in favor of endovascular coiling. Furthermore, after a review of the literature, Zaki Ghali et al 44 noted that ACoA aneurysms had significantly better cognitive outcomes if they were treated with endovascular coiling instead of clipping.…”

“…In addition, it aims to delimit the expectations for the treatment in a determined time. Thus, the evaluation stage has the 'golden' objective of understanding how cognitive sequelae manifest specifically in the life of the patient (Bolognani et al, 2007) and allow for the definition of the objectives of the rehabilitation, described in Table 2.…”
