Neuropsychological Outcome of Critically Ill Patients with Severe Infection

Abstract: Sepsis and septic shock represent important burdens of disease around the world. Sepsis-associated neurological consequences have a great impact on patients, both in the acute phase and in the long term. Sepsis-associated encephalopathy (SAE) is a severe brain dysfunction that may contribute to long-term cognitive impairment. Its pathophysiology recognizes the following two main mechanisms: neuroinflammation and hemodynamic impairment. Clinical manifestations include different forms of altered mental status, … Show more

“…For psychological effects, severe infection patients often require prolonged hospitalization and undergo various medical procedures and drug treatments. These processes can subject patients to considerable pain and discomfort, leading to anxiety, fear, and depression 49 . Furthermore, due to the prolonged recovery period required for severe infection, patients may encounter difficulties and challenges during rehabilitation, such as slow physical function recovery and decreased daily living capabilities, which can also have negative psychological effects.…”

“…SAE is most common in patients with sepsis in ICU and one of its most serious complications. Its pathogenesis has not yet been fully elucidated, but it may be related to autoimmune injury of the central body inflammatory reaction [ 20 , 21 ], a blood flow regulating mechanism for the damage caused by cerebral perfusion abnormalities and microvascular dysfunction [ 22 , 23 ], or a large release of cytokines in sepsis that leads to neurotransmitter imbalance [ 24 ]. Polyneuropathy is often more severe in the later stage of SAE development, which not only affects peripheral nerves but also rapidly causes cognitive deficits, personality changes, generalized epilepsy, and other symptoms.…”

“…Patients with verified sepsis according to the international consensus definitions of sepsis and septic shock sepsis-3 (Third International Consensus Definitions for Sepsis and Septic Shock, 2016), undergo clinical, laboratorial, radiological, and functional examination in order to identify nonspecific and specific functional and structural changes in the central nervous system inherent in acute cerebral injury, caused by a systemic infection. According to a number of authors, the absence of specific markers for the diagnosis of SAE complicates the diagnosis [4][5][6].…”
