Neuropathologic assessment of Alzheimer's disease

Abstract: In 1987, neuropathologists representing the charter group of CERAD clinics met to develop a practical instrument for standardizing the postmortem evaluation of the brains of individuals with Alzheimer's disease (AD) who had been enrolled in the multicenter CERAD study. At that time, standardized CERAD assessment batteries of the clinical and neuropsychological characteristics of this disorder had been established, and the need for a comparable evaluation of the neuropathologic findings had become apparent. Two… Show more

“…Although meritorious, these criteria were soon abandoned because, despite a very high sensitivity to diagnose AD dementia, they lacked sufficient specificity (Geddes et al 1997). In 1991, the Consortium to Establish a Registry for Alzheimer Disease (CERAD) proposed more specific diagnostic criteria by emphasizing the importance of neuritic plaques over diffuse plaques (Mirra et al 1991(Mirra et al , 1997. CERAD criteria use a semiquantitative score of the density of neuritic plaques in the most severely affected region of…”

“…The accuracy of diagnosing Alzheimer's disease (AD) has improved greatly in recent years and reportedly ranges from 82-93% using current clinical guidelines and assessment tools even for mild dementia (Becker et al, 1994;Berg et al, 1998;Gearing et al, 1995;Kukull et al, 1990;Mirra et al, 1997). Nevertheless, in Australia, 36.8% of respondents in a survey of general practitioners indicated that early dementia was difficult to diagnose (Brodaty et al, 1994).…”

“…The primary dementing disorders present in the remaining 25 patients included PD-related pathology (n = 9), hippocampal and entorhinal sclerosis (n = 4), miscellaneous neurodegenerative and other disorders (n = 6), and no significant changes (n = 6). 36 Thus, in a multisite setting which included experienced neurologists, the clinical diagnosis of AD based on the CERAD clinical battery (without reliance on the neuropsychological test results) was found to have substantial accuracy. At the same time it was obvious that concomitant pathologies could be present with AD, and that at least in some cases, clinically identified AD might be attributable to other conditions.…”
