Neuromodulation for Drug-Refractory Epilepsy and Chronic Heart Failure: Targets, Delivery, Composition and Titration

Abstract: Background: Vagus nerve stimulation (VNS) is used currently in the management of drug-refractory epilepsy (DRE), and is in development for treating chronic heart failure (HF). HF is accompanied by autonomic nervous system (ANS) dysfunction, consisting of sustained sympathetic hyper-activation and withdrawal of parasympathetic tone, and associated with progressive worsening of cardiovascular (CV) function and increased morbidity and mortality. We sought to compare how VNS is utilized for DRE and may potentially… Show more

“…The functional difference between these leads is in the polarity of the electrodes. In DRE and TRD, the cathode is positioned cranially, and in HF, the cathode is positioned caudally (Anand, Konstam, Ardell, Libbus, & DiCarlo, 2019;Ben-Menachem, 2002).…”

“…The therapies also differ in the frequency, pulse width, duty cycle, and current outputs that are utilized. Titration of VNS for epilepsy is empiric while a biomarker of autonomic nervous system engagement and cardiac control is used to guide titration of VNS for HF ( Inder S et al, 2019 ).…”

“…VNS can be delivered clinically using open-loop stimulation with a programmable pulse generator and an electrode lead that surrounds the cervical vagus nerve and requires no intraoperative mapping for placement ( Figure 2 ) ( Inder S et al, 2019 ).…”

“…Though the mechanisms of VNS for epilepsy are poorly characterized, reduction in seizure frequency and use of anti-epileptic drugs has been noted across trials (Ben-Menachem et al, 2015). The Food and Drug Administration has already approved implantable pulse generators and leads for safe and effective treatment of epilepsy, facilitating the adaptation of similar technology to alternate diseases (Ben-Menachem et al, 2015;Anand et al, 2019). Given the relevance of autonomic dysregulation in the development and progression of heart failure and arrhythmias, trials exploring the efficacy and safety of VNS for cardiovascular disease have received significant national attention since the initial report of its use for heart failure in 2008 (Schwartz et al, 2008;De Ferrari et al, 2011;Premchand et al, 2014;Gold et al, 2016;De Ferrari et al, 2017).…”
