Neurological problems that may occur in lung cancer are caused by both the cancer and treatment to be applied. Intracranial and extracranial nervous system metastases, cerebrovascular diseases, paraneoplastic neurologic disorders and treatment-induced peripheral and central nervous system diseases can be observed during diagnosis and treatment. The most common neurological problems of lung cancer are intracranial brain metastases. Making early diagnosis and administering treatment by considering the neurological symptoms in follow-up patients are important for both in terms of long-term preservation of neurological function of the patients and to provide longer survival.

Acute confusion is a common symptom of physical illness in the older patient. In the majority, it is transient and resolves on treatment of precipitants. In a subset of patients, however, neurological decline is progressive, raising concern about a serious underlying cause. We describe the case of a 71-year-old woman who developed progressive cognitive impairment following insertion of a permanent pacemaker for sinoatrial arrests. An initial diagnosis of delirium secondary to a urinary tract infection was suspected; however, the patient became increasingly confused despite treatment. Laboratory tests revealed serum anti-Hu paraneoplastic antibodies, and CT chest identified an occult lung tumour. Cervical lymph node histopathology confirmed a diagnosis of small cell carcinoma of the lung. Although a rare cause of confusion, paraneoplastic encephalomyelitis should be recognised early to allow timely identification and treatment of the associated cancer.

ÖZETOnkoloji pratiğinde nörolojik semptomlar genellikle direkt tümör etkisi veya paraneoplastik sendrom olarak karşımıza çıkmaktadır. Nörolojik semptomlara neden olan nadir hastalıkların tanısı özellikle metastatik kanser hastasında zordur. Burada kırmızı idrar ve nörolojik semptomlarla değerlendirilen bir metastatik akciğer kanseri olgusu sunulmuştur. Metastaz ve paraneoplastik patolojilerin ekartasyonu sonrasında bulgular porfiri tanısı ile ilişkilendirilmiştir. Tecrübemiz porfiri tanı ve tedavi hakkında klinik ipuçları verirken, onkoloji hastasında nörolojik semptomların ayırıcı tanısında tanısal yaklaşımın önemini vurgulamaktadır. Anahtar kelimeler: Porfiri; Akciğer kanseri; Nörolojik semptomlar ABSTRACT In cancer patient neurological symptoms are mostly due to direct tumor effect or paraneoplastic syndromes. The diagnosis of rare diseases causing neurological symptoms is challenging, especially in a metastatic cancer patient. Here we discussed a metastatic lung cancer patient who presented with red urine and neurological symptoms. After exclusion of metastasis and paraneoplastic pathologies, the findings were associated with the diagnosis of porphyria. While our experience is giving clinical clues about a rare pathology, it also reminds us the importance of work up in the face of neurological symptoms in oncology patients.
