Neuroimaging in Pregnant Women

Abstract: Choosing the most appropriate diagnostic neuroimaging study for a pregnant woman involves assessing the pretest likelihood of serious treatable neurologic disease, the diagnostic utility of various available computed tomography (CT) and magnetic resonance (MR) modalities, and the risks of each. Of these three elements-pretest differential diagnosis, utility of MRI and CT, and risks of MR and CT-the risk component is perhaps the least well understood by most physicians. We provide a basic review of the intrinsi… Show more

“…Gadolinium MRI at any time during pregnancy was associated with an increased risk of a broad set of rheumatological, inflammatory, or infiltrative skin conditions and for stillbirth or neonatal death. Iodinated CT contrast agents are likely safer than gadolinium‐based MRI contrast agents because of gadolinium accumulation in the amniotic fluid and fetal tissue 23 . In our experience, 2 patients performed non‐contrast brain CT, in 1 case with evidence of indirect neuroimaging SIH signs.…”

“…MRI is the preferred neuroimaging modality in pregnant women because it does not involve the use of ionizing radiation and allows for better soft tissue evaluation as compared to CT ( 58 ). There may be however some concern about obtaining an MRI in the first trimester because of the possible exposure of the fetus to the heat generated by the magnetic field and the strong noise exposure ( 59 , 60 ).…”

“…If headaches are prominent, different or new compared to pre-pregnancy headaches or accompanied by focal neurologic deficits, abnormal neurologic exam, or associated with blood pressure elevation, MRI of the brain without contrast should be considered to rule out the secondary causes of headache. 46 …”
