Neurodevelopmental outcomes at 2 and 4 years in children with congenital heart disease

Abstract: Despite significant correlations between 2- and 4-year-old test scores, many patients who scored in the average range at age 2 showed deficits at age 4. BSID-III scores at age 2 may underestimate delays. Therefore, longitudinal ND assessment is recommended.

“…In a study of CHD children, there was an overall good correlation between scores on the Bayley scale at age 2 and neurodevelopmental performance at age 4. However, several children who scored in the average range of the Bayley scales at age 2, had developed neurodevelopmental deficits by age 4 [ 167 ]. Abnormal cerebral findings in the prenatal and preoperative period may be better predictors of neurodevelopmental deficits.…”

“…Common concerns for young children with congenital heart disease referred to cardiac neurodevelopmental programs include delays in gross motor skills, expressive language, behaviour dysregulation, and feeding difficulties. 8,12,33,39 Young children also present to cardiac neurodevelopmental programs with dysregulated sleep patterns and with separation or medical anxiety. 24,33,39,45,48,76,77 Once children reach preschool, grade retention is often considered by parents, sometimes at the recommendation of schools or providers.…”

Grade retention recommendations are most common among children with histories of repeat or prolonged hospitalisations, and who have reduced stamina and endurance.…”
