Neues Konzept in der Therapie der distalen Tibiametaphysenfrakturen und wenig dislozierten Pilonfrakturen mit schweren Weichteilschäden

Abstract: The treatment of pilon fractures and distal metaphysial tibia fractures demands very high standards on the osteosynthesis material regarding the soft tissue and the essential joint reconstruction. The selection of the surgical entrance, particularly in case of a critical arterial or venous circulation and the possible irritation of the soft tissue caused by the osteosynthesis material led us to search for alternative osteosynthesis methods. After the elaboration of a pre-clinical study and good first results i… Show more

“…The XS nail ® (Intraplant ® ) for the treatment of malleolar fractures has a round profile and a diameter of 4.5 mm (Figure 1), or 3.5 mm for the XXS(L) nail [9,10]. Depending on the fracture to be treated, the XS nail is available with four to eleven locking holes with 9-mm spacing, with the two holes closest to the fixation having a longitudinal oval alignment.…”

“…• In osteoporotic bones, better stabilization using threaded wire and nail fixation than by using smallfragment screws. • Procedure protects soft tissue by using intramedullary positioning of the implant[1]. • Soft-tissue management of local complications made easier by intraosseous positioning of the implant.…”
