IntroductionStroke is a highly consequential medical condition, characterized by a substantial risk of death and disability. Based on the data of the World Health Organization (WHO), stroke was contibuted for 6.2 million of mortalities, the second cause of death globally. In particular, half of stroke survivors encounter challenges in performing daily activities, and the psychosocial aspects of their experience often lead to a diminished quality of life, contributing to conditions like depression. In addition for the impact of physical, stroke also can induce the cognitive barriers, impacted to an attention, orientation, retention, and cognitives functions. In addressing these cognitive challenges, particularly through cognitive therapy, was shown promising in reducing levels of anxiety and depression among post-stroke individuals. This literature research deals to examine research outcomes related to various post-stroke cognitive therapies. The objective is to describe the advantages and disadvantages of this therapeutic approaches, clarify the effectiveness in rehabilitate of cognitive and psychological consequences of stroke.MethodLiterature obtained through electronic media on Science Direct, Scopus and Google Scholar by used the keyword Cognitive Therapy Approach in Post Stroke Patients and it was found 5 articles that met with the criteria which had been published less than the last 10 years.Resultsthis study shows that cognitive interventions that duplicate memory, processing speed and attention can produce significant improvements in several cognitive domains. This therapy teaches compensatory strategies such as using a notebook or daily planner and analyzing tasks logically until activities are carried out well in daily life which involves exercises to increase attention and requires internal neurological attention. The function of this training includes visual and auditory skills, both of which are important for everyday training and information processing designed to improve retention and recall of information and improve memory. Attention and executive skill function are interdependent and have a significant impact on daily functioning. Therefore, exercises that improve attention, working and short-term memory can improve general mental abilities and improve a persons ability to process information.ConclusionThe results of this study indicate that cognitive therapy can help the patients to achieve the recovery optimally both in cognitive or emotional aspects.

Cerebral small vessel disease (SVD) causes lacunar stroke and intracerebral hemorrhage, and is the most common pathology underlying vascular cognitive impairment. Increasingly, the importance of other clinical features of SVD is being recognized including motor impairment, (vascular) parkinsonism, impaired balance, falls, and behavioral symptoms, such as depression, apathy, and personality change. Epidemiological data show a high prevalence of the characteristic magnetic resonance imaging (MRI) features of white matter hyperintensities and lacunar infarcts in community studies, and recent data suggest that it is also a major health burden in low- and middle-income countries. In this review, we cover advances in diagnosis, imaging, clinical presentations, pathogenesis, and treatment. The two most common pathologies underlying SVD are arteriolosclerosis caused by aging, hypertension, and other conventional vascular risk factors, and cerebral amyloid angiopathy (CAA) caused by vascular deposition of β-amyloid. We discuss the revised Boston criteria of CAA based on MRI features, which have been recently validated. Imaging is providing important insights into pathogenesis, including improved detection of tissue damage using diffusion tensor imaging (DTI) leading to its use to monitor progression and surrogate endpoints in clinical trials. Advanced MRI techniques can demonstrate functional or dynamic abnormalities of the blood vessels, while the high spatial resolution provided by ultrahigh field MRI at 7 T allows imaging of individual perforating arteries for the first time, and the measurement of flow velocity and pulsatility within these arteries. DTI and structural network analysis have highlighted the importance of network disruption in mediating the effect of different SVD pathologies in causing a number of symptoms, including cognitive impairment, apathy, and gait disturbance. Despite the public health importance of SVD, there are few proven treatments. We review the evidence for primary prevention, and recent data showing how intensive blood pressure lowering reduces white matter hyperintensities (WMH) progression and delays the onset of cognitive impairment. There are few treatments for secondary prevention, but a number of trials are currently evaluating novel treatment approaches. Recent advances have implicated molecular processes related to endothelial dysfunction, nitric oxide synthesis, blood–brain barrier integrity, maintenance and repair of the extracellular matrix, and inflammation. Novel treatment approaches are being developed to a number of these targets. Finally, we highlight the importance of large International collaborative initiatives in SVD to address important research questions and cover a number which have recently been established.

Vascular cognitive impairment is common after stroke, in memory clinics, medicine for the elderly services, and undiagnosed in the community. Vascular disease is said to be the second most common cause of dementia after Alzheimer disease, yet vascular dysfunction is now known to predate cognitive decline in Alzheimer disease, and most dementias at older ages are mixed. Neuroimaging has a major role in identifying the proportion of vascular versus other likely pathologies in patients with cognitive impairment. Here, we aim to provide a pragmatic but evidence-based summary of the current state of potential imaging biomarkers, focusing on magnetic resonance imaging and computed tomography, which are relevant to diagnosing, estimating prognosis, monitoring vascular cognitive impairment, and incorporating our own experiences. We focus on markers that are well-established, with a known profile of association with cognitive measures, but also consider more recently described, including quantitative tissue markers of vascular injury. We highlight the gaps in accessibility and translation to more routine clinical practice.
