Nerve supply of the breast with special reference to the nipple and areola: Sir Astley Cooper revisited

Abstract: Cooper in 1840 described mammary branches from the 2nd–6th intercostal nerves, and noticed that the nipple was supplied by branches which lay close to the surface of the gland. Eckhard (1850) divided the mammary branches into superficial branches to the skin and nipple, and deep branches to the glandular tissue and nipple, but many later authors ignored those findings. After the second World War, cosmetic surgery of the breast made further research critical, as surgeons strove to design operations which would … Show more

“…During an immediate reconstruction, there is also a submuscular dissection, followed by expansion of the prosthesis. The sensory nerve fibers that emerge from the intercostal nerves (17) are interrupted in both dissection planes. A study that compares patients undergoing mastectomy with or without immediate reconstruction would be of interest, to evaluate to what extent the reconstructive part of the operation affects the sensibility in the breast.…”

“…We kept the catheters from 2 to 7 days depending on the surgery. In 1840, Cooper described mammary branches from the 2 nd to 6th intercostal nerves (4,5). The pectoral fascia originates from the clavicle and sternum, covers PMM, continues inferiorly with the fascia of the abdominal wall to cover the rectus abdominis muscle and external oblique muscle and continues laterally with the fascia of the back to cover the serratus anterior muscle (SAM) (6)(7)(8).…”

“…The sensation was most affected above the areola. This might be explained by the surgical dissection, which is more extended in this direction both in the mastectomy and in the submuscular dissection, interrupting sensory nerve fibers emerging from the intercostal nerves (24) in two anatomic planes. In a study by Benediktsson et al.…”
