ВведениеТрансплантация почки -наиболее часто выполня-емая процедура по пересадке органов в мире. В 2014 г. было произведено 77 948 таких операций по всему ми-ру [1]. Начиная с 1960-х годов в мировую практику бы-ло введено выполнение иммунодепрессивной терапии у реципиентов, что позволило значительно повысить жизнеспособность трансплантата. Так, в настоящее время сообщается об успешном 5-летнем функцио-нировании пересаженной почки у 85,7 % реципи-ентов, получивших трансплантат от живых доноров, и у 73,5 % реципиентов, которым была пересажена трупная почка, на фоне выполняемой терапии, угне-тающей иммунитет реципиента [2]. Тем не менее про-ведение иммуносупрессивной терапии у данной кате-гории пациентов приводит к возникновению побочных эффектов, таких как инфекционные ослож-нения, сердечно-сосудистые заболевания, возникно-вение злокачественных новообразований. Появление злокачественных опухолей -одно из хорошо извест-ных поздних осложнений трансплантации органов, индуцируемое иммунодепрессивной терапией [3].Злокачественные новообразования кожи -наиболее часто встречаемые виды опухолей у когорты больных, получивших иммуно супрессивную терапию. Так, по данным Национального канцер-регистра Ирландии, отмечается 82-кратное увеличение показателей заболе-ваемости немеланомными кожными злокачественными новообразованиями у данной группы пациентов [4]. По данным других авторов, риск появления базально-клеточного рака кожи у больных, получающих имму-носупрессивную терапию, в 10-16 раз выше, чем в об-щей популяции [5]. Злокачественные новообразования почечной паренхимы составляют 4,6 % от всех новоо-бразований у данной группы пациентов, что несколько выше, чем в общей популяции, в которой доля новоо-бразований почек составляет 3,0 % [6]. В подавляющем большинстве случаев рак почки возникает в собствен-ных органах пациентов, однако в небольшом проценте (0,5-1,0 %) случаев он встречается в трансплантиро-ванном органе [7,8]. Наиболее распространенным гистологическим типом новообразований почечного аллотрансплантата является почечно-клеточный рак, светлоклеточный вариант [3,6].
