Neonatal sepsis contributes significantly to neonatal morbidity and mortality and is a major public health challenge around the world. Depending on the mode of occurrence, a distinction is made between maternal-transmitted infection and that acquired in the postnatal period. Although the etiologies maternally transmitted diseases are well understood, those of postnatal acquired infections are variable depending on the epidemiology of each hospital environment. On the one hand, risk factors for maternal-transmitted infections are maternal sepsis, prolonged premature rupture of membranes, chorioamnionitis, and bacteriuria in the mother during pregnancy. On the other hand, risk factors for postnatal acquired infections are prematurity, low birth weight, lack of hygiene, and invasive therapeutic interventions. The diagnosis is based on a series of anamnestic, clinical and biological features. Although the positive diagnosis is based on the isolation of the germ by culture on a body sample (blood, cerebrospinal fluid, urine, etc.); its low sensitivity leads to the use of markers of the acute phase of inflammation such as C-reactive protein, procalcitonin and interleukins. New molecular biology techniques are promising and offer precise diagnosis with rapid results. Empirical management is a function of microbial ecology while definitive treatment is guided by the results of microbial culture. This article presents the essential elements for understanding neonatal sepsis and discusses new diagnosis and therapeutic management. It offers a thorough reading based on the issue of infections in newborns.

Streptococcus agalactiae (GBS) là tác nhân truyền nhiễm hàng đầu gây nhiễm trùng huyết sơ sinh giai đoạn sớm. Tầm soát GBS và tiêm kháng sinh dự phòng ở phụ nữ thai sản có thể giúp ngăn chặn hữu hiệu tỷ lệ nhiễm GBS ở trẻ sơ sinh. Phương pháp truyền thống phát hiện và nuôi cấy GBS trên đĩa thạch máu rất tốn thời gian, công sức và độ nhạy thấp. Nghiên cứu này tiến hành tối ưu hóa phản ứng realtime PCR với cặp mồi và mẫu dò được thiết kế nhằm phát hiện gen đặc hiệu cfb của GBS. Các thí nghiệm tối ưu hóa được thực hiện trên chủng S. agalactiae ATCC 13813. Độ đặc hiệu của quy trình tối ưu được kiểm tra trên DNA của chủng Staphylococcus aureus ATCC 25923, Gardnerella vaginalis và Chlamydia trachomatis. Ngoài ra, quy trình tối ưu được thử nghiệm trên 30 mẫu dịch phết âm đạo-trực tràng của phụ nữ mang thai trong giai đoạn 35-37 tuần. Quy trình tối ưu đặc hiệu với chủng GBS, có độ nhạy 50 bản sao/phản ứng, độ chính xác 99.94%, và hiệu quả khuếch đại EA% = 94.5%. Trong số 30 mẫu thử nghiệm, 10 mẫu được phát hiện là có hiện diện của GBS, trong khi nuôi cấy truyền thống chỉ phát hiện 08 mẫu có GBS.

Objective. To determine the accuracy of 16S rRNA polymerase chain reaction (PCR) for the diagnosis of neonatal sepsis through a systematic review and meta-analysis. Methods. Studies involving 16S rRNA PCR tests for the diagnosis of neonatal sepsis were searched in the PubMed, Medline, Embase, and Cochrane Library databases. The methodological quality of the identified studies was evaluated using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2), and the sensitivity, the specificity, the positive likelihood ratio (PLR), the negative likelihood ratio (NLR), the diagnostic odds ratio (DOR), and the area under the curve (AUC) of operator characteristic (SROC) curves were determined. Heterogeneity between studies was analyzed by metaregression. Stata 14.0 and Meta-disc 1.4 software were used for the analyses. Results. This meta-analysis included 19 related studies. The analysis found a sensitivity of 0.98 (95% CI: 0.85-1), specificity of 0.94 (95% CI: 0.87-0.97), PLR of 16.0 (95% CI: 7.6-33.9), NLR of 0.02 (95% CI: 0.00-0.18), DOR of 674 (95% CI: 89-5100), and AUC of 0.99 (95% CI: 0.97-0.99). Metaregression analysis identified Asian countries, arterial blood in blood samples, and sample size > 200 as the main sources of heterogeneity. This meta-analysis did not uncover publication bias. Sensitivity analysis showed that the study was robust. Fagan’s nomogram results showed clinical usability. Conclusions. The results from this meta-analysis indicate that 16S rRNA PCR testing is effective for the rapid diagnosis of neonatal sepsis.
