Neonatal pneumothorax – an unexpected perioperative complication

Abstract: We report our experience of three cases of intraoperative pneumothorax in neonatal surgical patients. Following a review of the literature, we discuss possible causes for each case and methods of treatment. We emphasize the need for inclusion of pneumothorax as a cause for cardiorespiratory instability even when no predisposition is identifiable and highlight the need for prompt treatment to prevent serious morbidity and mortality.

“…Anesthesia-related barotrauma and pneumothorax are rare in children [ 1 ] and the first symptoms can go unnoticed due to mechanical ventilation and high fractional inspired oxygen (FiO 2 ). Pneumothorax due to barotraumatism in pediatric is relatively well-documented in the literature.…”

“…Kumar and Walker[ 28 ] reported cases of perforation of the airway caused by the bougie. Glaisyer and Way[ 29 ] reported 3 cases of pneumothorax during surgery in neonates, and no specific cause was determined, but 2 cases were reintubated using the bougie. In two patients with pneumothorax reported by Parekh et al [ 1 ], it was suspected that pneumothorax was caused by trauma to the tracheal wall that occurred during bougie-guided endotracheal intubation.…”
