Neonatal outcomes after introduction of a national intrapartum fetal surveillance education program: a retrospective cohort study

Abstract: Introduction of a national FSEP was associated with increased neonatal admissions but a reduction in intrapartum hypoxia, without increasing emergency caesarean section rates.

“…22 The INFANT study and the STAN studies, using computerized management and specialized surveillance, have failed to show clinical benefit compared to controls. [23][24][25][26][27][28] While other studies of EOD have been reported, it is not possible to compare these studies with our own, given the different requirements for inclusion. In several of these studies, we do not have assurance that the fetus was normal at the outset nor about the preventability of the EOD.…”

“…The uptake of FSEP has been associated with a reduction in perinatal morbidity and mortality 5,7 . In particular, since the introduction of FSEP there has been a reduction in the rate of neonates injured with hypoxic ischaemic encephalopathy in Queensland 5 and a halving in hypoxia‐related intrapartum deaths in Victoria 7 . A formal economic evaluation of FSEP may be worthwhile to better convince individual services of the value of paying for education for their staff.…”

“…Other future study aspects are whether extended CTG training could have an impact on neonatal outcome. Since the study by Byford and co‐workers , another retrospective study analyzing several measures of fetal outcome has been carried out with recently published results . When comparing before and after introduction of the Australian standardized education program for fetal surveillance, a national decrease in term perinatal deaths due to hypoxia as well as improvement in other neonatal measures has been achieved.…”

“…When comparing before and after introduction of the Australian standardized education program for fetal surveillance, a national decrease in term perinatal deaths due to hypoxia as well as improvement in other neonatal measures has been achieved. During the years following the education intervention, there have been decreased rates both of hypoxic ischemic encephalopathy and of Apgar scores < 5 at 5 min among babies admitted to the neonatal intensive care unit, compared with the years before the education program introduction .…”
