Neonatal cutaneous fungal infections

Abstract: Cutaneous fungal infections in neonates range from generally benign conditions such as congenital candidiasis and neonatal cephalic pustulosis to potentially fatal infections with opportunistic pathogens in very low birthweight or immunocompromised neonates. The prompt recognition and appropriate treatment of cutaneous fungal disease in neonates is critical to the prevention of adverse outcomes.

“…Itraconazole infusion appeared to be well tolerated in this population with a single adverse event (stinging at the site of infusion) deemed to be related to study drug administration. Based on the findings of this investigation, it appears that intravenous itraconazole can be administered to infants beyond 6 months, children, and adolescents using a weight-normalized approach to dosing. With both traditional and emerging fungal pathogens contributing to an increasing rate of morbidity, invasive mycoses remain a serious and potentially fatal complication for immunocompromised children (1,16,23,26,27,31). In recent years, a growing number of new therapeutic agents have found their way to market (28).…”

“…The maturation (i.e., keratinization) of fetal skin is functionally incomplete at birth and continues over the first few weeks of life [11]. Thus, all neonates are at an increased risk for cutaneous infections that may present as pustules, blisters and/or erosions.…”

“…Aspergillosis is the second-most frequent opportunistic infection, surpassed only by candidiasis (1, 3, 4). There are more than 1,000 species of Aspergillus , but A. fumigatus is the most common cause of colonization and invasive aspergillosis (2, 3).…”
