Neodymium:YAG laser posterior capsulotomy after implantation of AcrySof intraocular lenses

Abstract: The Nd:YAG laser posterior capsulotomy rate after implantation of AcrySof IOLs was relatively low; the cumulative rate was approximately 1% to 2% new cases per year over a 7-year period, with a plateau suggested at year 6.

“…In the literature, PMMA IOLs are associated with the highest incidence of PCO. 1,2,4,8,33 A recent review 30 confirms that a polished IOL optic surface plays an important role in reducing LEC cell migration and adhesion to the lens optic. Hence, the submicrometric morphology and regularity of the IOL optic surface should be an additional factor in the migration and adhesion of LECs to the lens optic.…”

“…3,4,23 The role of a square-edged IOL optic in reducing PCO rates, independent of the IOL material, is well documented. 5,14,15,20,24,25 However, whether PCO is influenced by the IOL material remains controversial, 2,13,14,20,23,26,28 mainly due to the complexity of managing long-term prospective clinical studies directly comparing the PCO rate after implantation of different types of IOLs.…”

“…Posterior capsulotomy was used as the measurement of PCO. Davison (2004) reviewed over 12 000 consecutive cases of patients who underwent phacoemulsification and AcrySof Ò IOL implantation from January 1995 to December 2002. The total capsulotomy rate in this population, which had up to 7 years of follow-up, was 6.1%.…”

“…To date, only improvement in surgical techniques, IOL designs, and IOL biomaterials 9 have substantially reduced PCO 10,11 ; however, they have not eradicated it, especially in pediatric and uveitis patients. [12][13][14][15] Moreover, surgical removal of the central part of the posterior capsule with a posterior continuous curvilinear capsulorhexis (PCCC), 16 first described by Gimbel and Neuhann, 17 does not prevent LEC migration, 18,19 especially in compromised eyes.…”
